Employee was attempting to set a post outby along B Mains intake travel way. While doing so, EE was struck on their left foot by a loose piece of the rib, which resulted in a fractured toe and a contusion on EE's ankle.

While reaching into the scoop bucket to get roof bolting plates a piece of rib 20" thick X 23" wide X 36" long struck employee on the back, pushing EE into the scoop bucket.

Miner was tramming scoop along #5 entry (travelway) of Black Mains approx. 1800 feet outby the section. A brow of rock popped free of the rib and struck a 5"x7" post that was 8 feet in length, causing it to fall into the kitchen of the scoop, striking the miner on the left side of his head.
