Employee stated was moving cutting slack out of the roadway when a small piece of rib rock fell out from the top of the rib and struck employee on top of right foot. IE sought medical attention on Friday 10/12/2018 where IE was diagnosed with a fractured bone in foot and was taken off work by IE's doctor.

Employee stated that as employee was walking to the #2 entry face in the last line of breaks employee stopped in the intersection at SS# 1400 to pick up a pop can. At this time a section of the inby rib sloughed off striking employee's upper back.
Employee stated there was a shuttle car parked in the heading on the #1 Section in the #4 entry. While the employee attempted to walk between the rib and the shuttle car, a section of rib fell out and struck employee on the left ankle.
INJURED WAS OPERATING MINER IN 4 LEFT. EE WAS SETTING MINER OVER FOR CLEAN-UP. EE WAS BACK OUT OF RED ZONE AT 4 HEADING FACE WHEN A SECTION OF RIB FELL STRIKING RIGHT SHIN. INJURED HAD A COMPOUND FRACTURE AT SHIN AREA.
INJURED HAD CLEANED GOB OUT OF UNCUT FACE. HE SET THE MINER OVER TO GET HIS INITIAL CUT. A PIECE OF ROCK FELL OFF OF RIB, STRIKING HIM IN THE LEG. THE WITNESS GOT THE ROCK OFF OF INJUREDS LEG.HIS LEFT LEG WAS BROKEN IN TWO PLACES.
INJURED STEPPED OUTBY TO RETRIEVE A SECTION OF LINE CURTAIN. WHEN HE BENT OVER TO PICK THE CURTAIN UP, A SECTION OF RIB 4" LONG, 5" THICK AND 9" WIDE STRUCK HIM ON HIS RIGHT SHOULDER. THIS LEFT REDNESS AND SMALL SCRATCHES TO HIS SHOULDER AREA.
INJURED WAS MOVING TRACKING CABLE UP. SETTING UP THE SPOOL OF CABLE, THE INJURED USED HIS FOOT TO CLEAR AWAY SOME RIB FLUFF. HE PUT THE SPOOL IN PLACE, AND A SECTION OF THE RIB ROLLED OUT PUSHING INJURED AND SPOOL OVER. STRIKING INJURED IN RIGHT KNEE.
