OPERATOR HAD BACKED THE HAULAGE TRUCK INTO POSITION ALONG THE WEST SIDE OF THE PIT TO BE LOADER BY THE SHOVEL. MATERIAL FROM TOP OF THE HIGH WALL SLUFFED OFF, HITTING THE SIDE OF THE TRUCK. AL THOUGH THE OPERATOR WAS WEARING A SEAT BELT, HE WAS THRUST UPWARD AGAINST THE SEAT BELT, CAUSING INTERNAL BRUISING TO HIS MIDSECTION
