Employee was pulling rib with slate bar and part of rib hit his leg when pulled loose. His first doctor visit and first day missed was 8/10/09.

Employee was installing roof bolts on the operator's side of the roof bolter when the left rib rolled off, pinning both legs under the debris. It bruised and caused swelling of the left leg.

Employee was changing scoop battery at Mains battery station. He walked around front of scoop bucket and rib roll caught foot against scoop bucket. He returned to restricted duty work on 1/18/06.

MEDICAL RECEIVED 6-21-02 ON INDICATES DEG SPINNING DEGENEATIVE MULTI LEVEL. HE HAS HAD OTHER BACK CLAIMS IN THE PAST FROM MEDICAL RECORDS.
