A piece of rock fell and struck the EE on the right arm causing a small laceration 2 CM in length. This incident was discovered during a Part 50 Audit. The incident was not reported to the Safety Dept. due to no loss time. Sutures makes it reportable.

The employee has had numerous back surgeries before this incident. The comp. board hearing concluded his incident would have contributed to his back problem. Discovered during Part 50 audit.
While roof bolting a small piece of sharp rock fell and struck EE on shoulder. This incident was not reported to mine management until 12/14/09.
