Showing all 2449-172E EE was collaring hole in back when piece of rock approx. 3" x 2" x 1" broke off, fell and struck him on "L" wrist between glove and arm guard. Stope was barred down very well.
Barring down loose material prior to bolting back in the 4300-275. Material came down and hit employee on right wrist (between arm guard and glove) causing laceration.
4600-165 - Rock bolting, rock came out of back hitting miner - right wrist.
Drilling and bolting with jackleg drill, miner had installed approximately 8 bolts when he started to drill the next hole. As he was drilling a rock loosened from drilling, falling and striking "L" thumb. Employee was wearing gloves and long sleeve shirt at time of accident.
Miner was in process of bolting, drilled for bolt, noticed loose area, left machine and steel in last hole drilled and barred down loose material. Loose came out of hanging and hit machine, then came down and hit employee on "L" thigh. Loose measured 12" x 8" x 4". Bruise to "L" Thigh.
Employee barring down, a rock that was being barred down came out of the back and struck employee on "R" elbow.
4600-146E Ramp. On June 4th EE was rock bolting behind brow, rock came from behind brow down rock bolt steel hitting "L" hand. EE went back to clinic on June 24th for follow-up. X-rayed again. Fracture to "L" distal radius, cast was applied
EE started shift by barring down his stope 210S from raise to face. Rock came out from behind roof bolt and holy board, hitting EE on "R" Shoulder.
EE was reaching for his jackleg to take to face, when he was struck on top of head by unknown object (EE had hardhat on at time of accident). There were two basketball sized rocks in walkway, probably was hit by one or both of them.
Bolting in 4600 Vent Raise rock came from behind ladderway and hit EE behind "L" knee
EE was lacing the outside of the raise with lagging. He raised a piece of lagging to put in place and the lagging came in contact with the back, this caused a rock to fall out of the back striking the EE on the back of the neck causing the laceration.
Employee was installing ground support. As he was tightening a rockbolt, a rock fell striking him on the upper leg causing a laceration requiring sutures.

Employee was barring down in a drift heading onto previously barred material. A rock deflected and struck the employee in lower leg resulting in a laceration requiring sutures.
