A piece of rock struck a miner on the left hip/back while drilling a hole to install a roof bolt. The vibration of the rotating drill steel in adverse conditions caused the rock to fall. The miner attempted to get out of the way when EE noticed the rock moving.

Showing all 16Employee was on operator side of the bolter at the inch trams when a section of rock and coal fell from the rib striking EE in the right foot causing multiple fractures to EE's foot and toes. Rock was approximately 14" wide by 8" thick by 6' long.
Fletcher Roof Bolter ATRS knocked down a rock from the roof. When the rock fell to the bottom it landed on another rock and broke apart. One of the smaller pieces flew up and struck the bolter helper in the chin. Employee was standing approx. 12' behind the last row of bolts. Employee received a cut on the lower lip/chin area which received 4 stitches.
Employee was in intersection waiting for the belts to come back on when a section of roof rock fell striking ee in the left foot causing a crushing injury and amputation of three toes. the rock measured 37" wide 67" long and 11" thick.

A piece of rock measuring 10" X 16" by 4" thick fell from the roof and contacted the metacarpal glove on right hand. The rock was sharp and it cut through the glove. Employee received a laceration on two fingers on right hand.

A miner was injured while installing roof bolts in adverse conditions in the #5 entry of the 7 mains section. A piece of roof rock came loose and fell on the bolter head. The piece of rock ricocheted off the head and hit the injured miner on the right side of his face causing a laceration.
