Showing all 4As the employee was attempting to hang a ventilation tube, a slicken sided piece of roof rock 7-8-inches in diameter and 4-inches thick, fell and struck the employees hard hat. The hard hat cut the employees forehead.
The employee attempted to remove a concrete stopping block from the roof by stepping up onto the rib mesh. When he hit the block a piece of rib fell behind the rib mesh and struck the emploee's fingers which were through the mesh.
As a timber was being set for rib support, a piece of top draw rock was dislodged and struck the injured's left foot.
