A continuous miner operator was cutting coal out of the #7 heading while positioned on the lefthand inby corner of the #7 entry. One piece of the corner that consisted of coal and rock sloughed off striking EE on the left shoulder and back area. The employee was placed off of work beginning 7/21/25.

Employee was bolting in the #6R cut. Removing the steel from the outside bolt on the fifth row when a rock measuring 29.5'' X 28'' L X 3'' Thick fell striking the employee's right pinky finger. The entry was 19.5' wide x 73'' high and damp. Fracture to right Pinky requiring surgery.

The Employee Knelt down to change the water filter on the RCM SN JM7225 in the #5 entry on 2 section. Employee had left hand on top of the miner when a rock measuring approximately 12" by 9" by 7" fell striking the employee's left pinky finger. The area was approximately 6 feet high. This incident resulted in a partial amputation.

The Employee was observing the bolt crew drill a test hole when a piece of rock approximately 3' L by 4' W By 1" to 6" thick fell striking EE on the hard hat, causing a 1 inch laceration on the back of EE's head. the entry was 18'6" wide by 62" high and damp to wet. The employee received 3 staples.

An offside roof bolter operator was injured while attempting to remotely extend EE's ATRS Pad out. While positioned to extend EE's ATRS pad out manually, a piece of draw rock fell on the operator side ATRS causing the offside of the ATRS to raise and strike the offside employee in the face area. This resulted in a concussion and damage to teeth.

Employee checked the top before bolting and noticed a loose rock and tried to pull it. When the rock would not come down, EE drilled the hole for the bolt and when EE inserted the bolt into the hole and went to push the bolt up into the roof, the rock fell and hit EE's left pinky finger causing a laceration.

The offside roof bolter operator was in the process of installing a 2nd roof bolt in the 2nd row when they noticed a piece of draw rock moving out of the corner of their eye. A piece of draw rock measuring approximately 45" by 25" by 1/2" fell striking them just below the left elbow resulting in a cut that required stitches.

As the employee turned to get hat a second piece fell from the mine roof striking the employee in the top of the head causing a laceration. The employee went to Raleigh General Hospital and received 3 staples. Employee was off work for 1 day.

Employee had just finished scaling top and had returned to the drill station to start bolting, as EE reached for a bolt a piece of rock 14"x19"x1 1/2" fell and hit the top of EE's right hand. EE was taken to hospital, x-rays were negative. EE had to wait to be seen by orthopedic and the orthopedic has EE off for 2 weeks for physical therapy.

The operator was transported to Raleigh General Hospital where x-rays showed a broken fibula, broken pinky toe, and cracked ankle. Operators return to work is unknown at this time. Operator worked for Lightning Contracting Services MSHA ID-8CV.

Employee was scaling the top when a rock fell and hit him in the left leg. X-rays taken at St. Joseph Hospital were negative and the employee returned to work on his next scheduled shift. The employee continued having pain and was seen by an orthopedist, which recommended surgery on September 25, 2014. *The Employee was operated on September 25, 2014 and will be off work.
