Showing all 6The shear cut through a fracture zone where sandstone is falling out 3-4', shield 15 to 27. The face sloughed out enough that a tall pile of material was between the conveyor and the face. There was minimal material in the conveyor. A rock fell out of the roof in the face and hit the pile of material. It fell at an angle that allowed the rock to come over the pan striking employee.
A rock fell against a longwall shield and pushed it forward and employees leg got caught between the toe of shield and pan line.
EE was setting shield sprag, when the initial cave behind longwall happened. The air pressure knocked EE off EE's feet and EE landed on EE's back on shield toe.
The employee was dropping ventilation from the roof for re-use. A piece of rib coal and roof rock fell behind the employee and knocked him down. The rib coal/roof rock was 2-foot thick 3-feet long and 2-feet high.
While attempting to set a timber the employee was holding the timber. When the cap piece was struck with an axe a piece of roof rock 10-inches X 10-inches X 3-inches fell and struck the employee on the head and right shoulder.
