employee was bolting top and when pulled forward to install the next row of bolts a piece of rock fell from between last two rows of bolts installed and struck employee on head and foot
IE was setting up to start loading with the CM. IE had adjusted IE's tubing for ventilation & as IE walked outby towards the end of the CM a rock hinged off of the last row of roof bolts & knocked the tubing down. The tubing knocked IE down & the rock flipped over onto IE's left ankle which caused the injury.
EE was pulling excess curtain from the mine floor to advance curtain on bolt cycle in the face. A rock measuring approximately six inches wide by seven inches long by three inches thick dislodged from the mine roof and struck ee on left shoulder. EE finished shift but went to the emergency room later in the evening with complaints of shoulder pain.
Employee was on-shifting the 9N#3 entry from the mouth to the 25 break t-car. The employee had gone through the first check curtain located just inby #4 break and was headed towards the 2nd check curtain when a rock, approximately 3' x 3' x 1' thick, fell from the top and struck the employee on the hard hat driving ee to the ground.

The employee was constructing a stopping at 59cc #1 Section between #7 and #8 entries. The area was 9' 3" high and 18' 8" wide. EE was standing on a ladder to place another block on the stopping. As EE placed the block, a section of rock measuring 24" x 13" x 1" fell out and struck EE on top of the right hand. The injury took 7 stitches to close.
Employee was installing permanent roof support when a rock measuring 12" x 12" x 3.75" fell from the mine roof and struck the employee in the top back of EE's hard hat.
IE was helping on the longwall recovery face. EE was unhooking the shield to walking shield hoses. EE was standing between the shield & the crib when a 2'X4"X2"-4" thick rock fell on EE. Employee was getting ready to drill a roof bolt hole in the mine roof. A piece of rock measuring approximately 1'L x 1'W x 3"T fell from the mine roof when the drill steel touched the mine roof. This resulted in a laceration to EE's right forearm and took 4 staples to close the wound.
