A miner was in the nest of an Alimak Raise putting together a 2" valve for the hose reel when a rock came out of the raise, bounced into the nest striking the miner in the leg causing a laceration requiring sutures.

While pinning screen the miner was contacted by rocks falling from the back. EE was contacted by the rock causing fractures to EE's left leg, right ankle, and spine. The ankle injury required surgery.

Initial report was employee was contacted by a small rock that fell through hog wire on left shoulder. Employee worked EE's way through levels of care and was finally diagnosed with a partial tear of EE's rotator cuff on 12/22/23.

Employee strained neck while on a mechanical bolter when a slab fell from the face in heading EE was bolting. The slab struck bolters boom causing the bolter to bounce resulting in the strain. Was seen at ER on 8/29/23 and issued full release. Was seen again on 9/6/23 for continued neck pain at which time restrictions were issued making this reportable.

A miner was bolting with a jackleg when a piece of loose fell from the spring line. The loose rolled down EE's drill steel, bounced off the welded wire and struck EE on the left shoulder. The impact knocked EE down and the piece of loose landed on EE's right leg. EE suffered a broken left collarbone and a broken right leg.

Employee was scaling down a work area when one of the rocks EE brought down contacted EE's left hand resulting in injury. Seen by site medic referred to ER, x-rays were positive for a fracture to the left middle finger.

During the bolting cycle a rock contacted the miner in the left knee causing ruptured ligaments. The miner was seen at the site medic and referred to an orthopedic provider. Due to the swelling the MRI was not completed and read until 3/15/23. On that date the diagnosis of the ruptured ligaments was determined, and surgery scheduled.

Employee along with two other employees were in the process of loading a face round in a production heading when a rock fall occurred at the face contacting the miner in the left ankle resulting in swelling. Seen by site medic, referred to ER, x-rays were negative, referred then onto an Orthopedic specialist, restrictions were issued.

Employee was taken off work on 12/12/2022 from an injury EE sustained on 11/28/22 from rock contact to EE's left shoulder during the loading process with two other miners. After several doctor visits it was determined EE received deep tissue bruising and a possible slight tear to EE's rotator cuff tendon.

Employee was bolting using the CMAC bolter when small rock fell through the wire mesh and bounced off the drill mast contacting EE in the left side of mouth breaking a tooth. Seen a dentist and repairs to tooth were made, no work restrictions were issued making this a medical reportable incident.

Upon driving a split set bolt, employee reached up to turn the throttle off on the jackleg machine, when a rock contacted employee's gloved left pinky finger resulting in an avulsion. Seen by medic, referred to ER, X-rays were negative, two sutures were required & restrictions were issued.

Employee was installing last ring of rock bolts using a CMAC bolter when a rock fell from the face, as the rock was falling employee was turning to exit bolter deck, rock landed on the ground tipping over contacting employee's right foot that had slipped while employee was exiting the deck. Seen site medic, referred to ER, x-rays were positive for a fracture to little toe.

Employee scaling the rib near the face & as rock was scaled off more came down than expected & in an attempt to keep a plate sized rock from hitting employee in the torso employee swatted it away with gloved right hand. In doing so employee received a laceration to the right palm which required sutures, a full work release was issued making this medical reportable.

While bolting EE noticed loose forming in new wire EE was installing, while scaling the accumulation of rock out of screen, a small rock EE scaled out contacted the rib & bounced contacting EE on a tooth causing a chip, EE opted to not have it fixed at the time. On 11/11/2019 company was made aware EE had tooth fixed 10/15/19 making this a MR no days lost.

Employee was driving a micky bolt with a jackleg machine when a small rock broke from the perimeter of the bolt hole falling thru the wire mesh contacting employee in the upper lip resulting in a laceration. Seen site medic, referred to ER, sutures required, Full work release issued making this a medical reportable.

While bolting, employee felt something contact middle back. EE looked back & saw a small rock & continued to work for several hours. Later EE mentioned the incident to partner, who took a look at EE's back, at which time EE realized there was a laceration. Seen Site medic, referred to ER. Staples were used to close cut & full work release issued making this medical reportable.

The miner was loading round when a small rock fell and caused a laceration to left cheek. EE was seen by the site medic and referred to Pioneer Medical Center to have cheek evaluated. EE received sutures and was issued a full release.

During the bolt up cycle an employee was tightening a split set bolt to the rock a 2" by 4" rock broke away & fell thru the wire mesh contacting EE in the safety glasses causing the glasses to push down on EE's face & nose resulting in a laceration on the bridge of EE's nose, sutures were required, full work release was issued making this a medical reportable.

After completing a work place inspection an employee was in the process of loading back-stope blast holes along with partner using a pneumatic powder loader & while inserting the static free hose into a blast hole a rock contacted employee's hardhat causing lacerations, concussion, and a neurological injury.

While bolting up last ring of the round a small rock came through the screen and bounced off the jackleg muffler. The rock contacted the miner in the nose and lip causing a laceration. Miner was seen by site medic and referred to a medical center where the miner received cosmetic sutures to close the laceration.

The miner was bolting up last ring with a jackleg when EE's right hand was struck by a rock from the footwall/ back contact. EE was seen by the on-site medic and transported to Livingston Medical Center to have EE's hand evaluated. EE was diagnosed with fracture to a metacarpal and issued restrictions.

Employee had just set another 4' x 8' section of screen on the bolter's stinger to continue covering the back. As EE was walking away, a rock fell onto a partially secured screen & sprung off the end of the wire catapulting the rock 10' at & contacting the miner in the shoulder and forearm as EE had back to it. X-rays were positive for a fracture to right forearm.

When pulling jackleg off the bolt, a rock fell from corner of the face & footwall contact at shoulder height. Rock contacted employees left side & when it landed on ground fell onto left ankle. Seen by medic, referred to ER, X-rays positive left ankle fracture, restrictions issued.

EE was bolting third row of bolts in the back when a rock came out of the back and bounced off the ground contacting EE's foot. EE was transported to Pioneer medical Center and diagnosed with a bruised foot and given restrictions causing a lost time accident.

Tripping rods out of a 90 degree up hole, when the bell of the rods caught the wire and a piece of loose rock hit the back of EE's hard hat and then another hitting EE in the face causing a laceration under left side of nose on upper lip. The laceration is approximately 1 inch long.
