Scoop operator was hand shoveling along the rib at the face of the crosscut B-4 production hole on the return side, when a piece of roof approx. 15" wide, 4' long and 10" thick fell from the roof in between bolts and struck the operator on the right shoulder, dislocating his shoulder.

While bolting a face cut. The bolter operator was in the process of preparing to drill the hole for the inside bolt on his side of the bolter. When the bit touched the roof, a piece of roof rock 1-2" thick, 2' wide and 5' long fell from the roof. It hit the support arm of the ATRS and slid down it, hitting the bolter operator on the right side of his head, shoulder and chest.

EE was bolting in the #2 cross cut from Entry 4-5 in the 2 Rt. Room off of 2 Rt. Submain when a rock fell aprox. 18" X 24" X 2" thick. As he turned away he twisted his right ankle and was struck by the rock. Accident was not reported to management until 5/17/11.

EE was shoveling the rib in the 4-5 crosscut from #4 entry of the 1-Rt panel at Break #4 when a A-frame from the roof in between bolts (approx. 0-4" thk x 2' wd. x 3' lng.),fell, striking him in the back of the head and left shoulder.

Traveling in the intake on a mantrip, the miner was struck on the forehead above his left eye by a piece of draw rock that fell from in-between roof bolts as he traveled by, causing a laceration which required four (4) stiches.

Employee had just taken the scoop off of the underground scoop charger when a 2' x 2' x 4" piece of draw rock dropped from inbetween the bolt pattern and caught him in the front left shoulder area and chest. Area was scaled to correct any future problems.

THE CREW WAS CLEANING UP A ROOF FALL, A 4" THICK SLAB OF ROCK 4' BY 3' SLID OFF OF THE ROOF FALL AND HIT EMPLOYEE IN THE BACK OF HIS RIGHT LEG SEVERAL INCHES ABOVE HIS ANKLE. THE ROCK BROKE HIS ANKLE AND KNOCKED HIM DOWN.
