Employee was installing a roof bolt off of the miner when a piece of rock fell from the roof (apex) striking the employee. The rock measured approximately 2ft long by 2-3ft wide by 6 to 10inches thick. The rock fell striking the employee on the lower back and right ankle. Diagnosed with a fractured bone in right ankle

Employee was roof bolting off of the side of the miner when a piece of rock fell from the roof at the edge of the roof mesh and struck EE on the side/back of the neck head and shoulder. Piece of rock was approx. 16inches long by 6 inch wide by 2 inches thick.

Employee was removing debris when a piece of rock fell between two shields and struck EE in left hand. EE was wearing EE's metacarpal gloves at the time. Injury was not the result of a training or compliance issue.

Employee was working on a tailgate sprocket in H3 longwall. Employee was struck by a rock on top of their head while standing under 257 shield. Rock was 8"x8"x6" and believed to have came from the edge of the supported tailgate entry and bounce off of 258 shield tip striking them. Employee was diagnosed with a concussion and a T12 fracture.

Employee was assisting changing bits on the tailgate shearer drum. When a rock fell from the tip of the shields and struck the employee on the back of the head and neck causing a laceration. The employee received staples to close the laceration and is being treated for lower back issues.

Employee was working on the #1 West Section building a stopping between #5 & #6 entry. While building the stopping a piece of draw rock fell measuring 25"x12"x1" thick striking the employee on the left forearm.

Employee was side bolting on the miner in the H5 section when a rock dislodged from the mine roof striking the employee on the top of left shoulder. The rock measured 8"x8"x4', no broken bones or tears were found.

Employee was operating the right-side roof bolter on #1 West Section in the #7 right entry. While, inserting drill steel in the pot to install extra bolts a piece, of rock fell measuring approximately 20''x 12''x 5 inches thick striking the employee left hand causing a fracture to the base of 4th distal phalanx.

Employee was installing a "tilt out" bolt off of the miner in 13L section. A small piece of roof rock vibrated loose from the roof corner, while employee was drilling the hole. The rock fell striking the employee in the nose causing a laceration that was treated with 9 stitches. Employee did not miss any scheduled work as a result of this incident.

Employee was repairing a pan jack hose fitting on the miner in 3K when a piece of rock fell from the roof between two roof straps striking EE. Employee suffered a laceration to the right ear as a result of this incident. Stitches were used to close the laceration. Employee did not miss any scheduled work as a result of this incident.

EE was roof bolting in the #6 entry on #1 West Mains Section. While, bolting EE noticed some draw, rock so EE proceeded to scale the mine roof with a slate bar. As EE was scaling, a piece of rock fell behind EE, striking EE's left leg causing a broken, Tibia and Fibia.

While side bolting on the left side of the miner in the 1NTG Section, a rock fell from the roof and broke apart on the rock deflector. A piece of the rock that broke apart fell outby and struck the right side of employees head / neck area. Employee was diagnosed with a concussion.

Employee was side bolting on the miner in 2B section when a piece of rock fell from the roof and struck EE on the back of the hard hat causing the hat to fall off EE's head. Smaller debris fell from the roof and caused lacerations to EE's head and neck. Received five staples on the back of the head and three stitches on the neck.

Employee was installing a tilt out bolt off of the miner when a piece of rock fell and pinched EE's left index finger between the rock and the rib door that EE had their hand on. X-rays revealed a small fracture to the tip of the left index finger. No lost time as a result of this incident.

Employee was bolting on the left side of the Joy miner at 13+28 in H4 section. A rock that measured 3"x4.5" up to 8" thick fell and landed on the rib protector door, sliding down and hitting the employee on the upper back. Employee returned to work on 7/21/23 and was then diagnosed with concussion like symptoms on 7/24/23. No other part 50.20-5 apply.

Employee was standing on #4 shield using that shields PMCR to pull in #2 shield. A rock became dislodged and fell between #3 and #4 shield. Striking the employee on the left pinky finger resulting in 3 sutures.

Employee was rib bolting in 9 South Mains when a rock fell from the rib and glanced off EE's back. Employee felt pain in left shoulder after the incident.

While pre-shifting 11L miner section, a piece of rock fell from between two roof straps and struck the employee on the back/head and left leg. Employee was initially treated and released with no restrictions, but has since been taken off work for treatment. Employee began missing work days as a result of this incident on 3/6/2023

Employee was side bolting on the miner in the 2B section when a small piece of rock (4"x4"x2") fell from the mine roof and struck employee on the left ear, causing a laceration requiring 7 stitches.

Employee was cleaning rock out of the cable trough on the longwall panline/ A piece of rock fell from between the shields and struck their left hand. Employee went to the hospital after the shift and was diagnosed with a fractured bone in the hand.

Employee was sitting on the cable trough waiting for tools to service the longwall shearer in 9L section. A small piece of rock fell from between the shields, bounced off of the head gate drum and struck the employee on the right leg. This caused a laceration that used stitches to close. Employee did not miss any scheduled work as a result of the injury.

Employee was side bolting on operator side of Continuous Miner located in the H1TG section. The employee had next 6' bolt to install resting up against rib protector door. When reaching for said 6' bolt with left hand, a rock from the top fell striking the left hand against the top of the rib protector door. EE was diagnosed with broken finger.

Employee was bolting off the miner when a piece of rock fell striking left calf. Employee suffered a laceration and received stitches to repair.

Employee was side bolting along the Continuous Miner located in the H1HG CM Section. EE was in the process of hanging rib mesh when a rock 1x2x2 fell from the top striking the injured on the nose. EE received four sutures.

Employee was drilling when a Rock (Fire/Clay/Mud Mixture) fell from below Panning tin and struck EE on the hard hat and glanced off EE's left shoulder. EE was done collar piping and was drilling on the 4 row when the accident occurred. Rock measured approximately 16"x21".

Employee was on the face at the G2 longwall section at around #4 shield, when a rock approximately 2' long x 1 ½' width x 6" to 12" thick fell between #3 shield and #4 shields, reflecting off the headgate drive and striking EE on the right leg. Incident resulted in fractures to the fibula and tibia in EE's right leg.

While bolting off of the miner, a rock fell from the top and struck a hydraulic hose while the employee was raising the head. The fitting on the end of the hose broke and the hose struck the employee on the right foot. Employee was taken off of work for a contusion. No laceration, nor fracture.

While operating the miner, a rock came down between the mesh and struck EE on the left middle finger. EE received ten sutures to the finger.

While side bolting off the non-tube side of miner. Employee was setting the roof strap when a rock approximately 5'x4'x3' fell from the roof hitting the employee in the back of right knee/calf, pinning leg between the miner step and the rock. Employee was diagnosed with a contusion of the right lower leg and Edema & Bruising.

Employee was walking to move a vent tube along the continuous miner, when a rock measuring 6' in length, 3' wide, and 18" in thickness fell from the mine roof. The rock struck EE on the back causing fractures to EE's femur, ribs and vertebrae.

Employee was operating a gopher drill on the 5A Section belt when a piece of rock came from the mine roof and struck employee's right hand causing a laceration to the right middle finger. Employee received 13 stitches.

While driving a haulage jeep, a piece of rock fell from between the roof straps and struck the employee's left hand causing a fracture and a laceration to the left hand.

CB Mining employee was assisting with the assembly of an Armored Face Conveyor Chain on the head gate. While in a prone position a rock approx. 24" X 18" X 3" dislodged from the roof and fell approximately 4 feet striking the back of the right knee. Employee experienced sharp pain and knee was immobilized.

Employee was bolting off of the miner when struck by a piece of rock that caused them to fall against the miner. Employee injured their right shoulder and was diagnosed with a bruised back. Employee was taken off of work.

While working in the G1 #1 entry, employee was side bolting on tube side of miner, when a rock fell from top and struck employee on neck and back of head. Employee was diagnosed with a concussion.

Employee was walking through a partially hung check curtain and a small piece of rock fell from the roof/rib and struck EE on the side of the face causing a laceration. 6 Stitches to close. No work missed as a result of this incident.

Employee was roof bolting on the return side of the miner in the 5A section. While drilling a bolt hole a rock approx. 4' x 3' x 1" came out of the roof area and struck EE's right lower leg causing a laceration, requiring sutures.

While at the face a rock fell and hit EE's right leg. Original incident occurred 7/26/18 - first day missed 9/13/18.

Employee was roof bolting on the continuous miner in the 2 East Mains Section when a rock measuring approximately 22' x 18' x 6' fell from the rib above the binder and rib protector and struck EE on the back of the neck and shoulders. The rock knocked EE to the ground and EE was allegedly unconscious for 15-30 seconds.

Employee was rib bolting on the continuous miner in the G1 HG section. While separating the drill steels with hands a rock fell from the mine roof and struck EE's left hand causing a laceration to the left ring finger that required 5 sutures.

Report was, while retrieving a bolt from the miner, a piece of rock fell from the roof and struck ee on the head and back of the neck. Initial doctors slips and evaluation reports were not consistent with this incident description. An independent medical evaluation revealed that the symptoms were related to the incident description, therefore this incident became a LTA on 3/28/18

Employee was setting prop and rock fell from top and hit employee on head causing injury.

While employee was changing bits at the headgate a rock roughly the size of 12" Long x 6" wide x 2" Thick fell out between the #19 and #20 shield hitting the employee on the top of the right middle finger causing a cut/laceration; 6 sutures

Employee was working on the F-27 Longwall face when a rock fell from between the shields and grazed the left side of face causing a laceration to left cheek that required 5 sutures to repair.

While the employee was operating the remote control for the Continuous Miner, a rock fell from the roof and struck employee's left thumb. Diagnosed with a hairline fracture of the left thumb 7/27/17.

Employee was assisting with changing flights on F26 Longwall face at the headgate. A rock fell from the roof / face and struck the injured on left foot. The shields were pulled in to the face in this area. Diagnosed with fractured left 5th toe on 7/24/17.

Employee was center bolting in F28 HG. EE was installing roof bolts in a turn out area. After drilling the hole, EE pulled out EE's steels and a rock fell from the roof and hit EE's left arm. Rock was approximately 2'x3'x3.5". It caused a laceration to EE's left forearm which required eight sutures.

Employee was side bolting on the miner in 2 East Mains. They were beginning the 4-5 crosscut at 86 wall. After they installed their first strap, the side bolter traveled outby just as the top inby corner fell out. The rock landed on their left foot. Diagnosed with a dislocated foot.

While side bolting, employee was raising a roof strap to the roof when a piece of rock 20"x14"x6" fell out from the roof and hit employee on employee's right knee. When employee fell, employee caught self and jammed employee's left wrist. Diagnosed with a blood clot in employee's right leg on 6/1/17.

Employee was side bolting in E29HG and while raising the steel to the roof a rock came out from the roof, bounced off the strap and struck the employee. Rock hit employee on the left side of employee's face and employee received two stitches above employee's left eye.
