I NAME FOREMAN WAS SUPERVISING A COAL CREW THAT WAS MOVING EQUIPMENT BACK TO THE WORKING FACT. WHEN A SINGLE PIECE OF ROCK 1" THICK 1 FOOT LONG AND 1 1/2 FEET WIDE FELL OUT OF THE ROOF BETWEEN A ROOFBOLT AND THE RIB STRIKING HIM ON THE BACK OF HEAD.
