A PIECE OF SLATE TYPE ROCK FELL FROM OVER HEAD CAUSING HIM TO DODGE HIS HEAD SIDEWAYS AND HE STRUCK THE CORN ER OF A SIDE LIGHT ONT HE MACHINE WITH HIS RIGHT CHEECK OF HIS FACE. RESUTLING IN A CUT APPROX 1/2 INCH LONG.

I.NAME THE MINE9 MACHINE OP WAS INSTALLING CUTTING BITS ON THE MINER WHEN A PIECE OF SLATE FELL FROM BETWEEN THE ROOF BOLTS & STRUCK HIM ON HIS SHOULDER & UPPER BACK. HE COMPLAINED OF PAIN IN HIS SHOULDE9 & DECIDED TO GOT TOT HE HOSPITAL FOR X-RAYS.
