The EE was bolting on Main West Haulage at 106 block. While EE was changing steels a rock fell from the mine roof and struck the EE on top the head. EE finished EE's shift, and went for medical treatment the next day.

While mining in the 8th South longwall panel #10, there was a bounce and outburst of coal at the tailgate at shield #136, which resulted in the mining equipment being down for more than 1 hour. One person was injured and received medial treatment (stitches).
The employee was operating a locomotive, when the employee came to a stop and a rock unexpectedly fell from the mine roof and struck employee in the head and shoulder. It did not include any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
Injured employee was installing roof bolts from the right side of the continuous miner while slabbing part of a belt trench. As EE started drilling a bolt hole, a piece of rock approximately 2 inches thick by 16 inches in diameter fell and struck EE's right shoulder. Employee stated that the roof appeared good and EE had installed 7-8 bolts without any issue.
Employee was setting a beam across number 10 belt, after setting the beam and while gathering up tools, a piece of slate broke loose above the beam and struck them on the hard hat, sliding down back, compressing neck area.

Employee was working near the crusher area of the 7 West Longwall, when a rock fell from the top, striking EE's right shoulder causing a contusion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
The employee was rib pinning on the 1WHG Section. EE was reaching for a rib strap when a piece of roof material fell from above the vent tube striking the injured in left forearm. The injured received 6 stitches. No laws or company polices were violated.

Employee was walking across the track to get in the clear when a wooden roof plank unexpectedly broke. This caused roof material to fall, striking head and neck. It did not involve any equipment or compliance issues. It also did not involve any job skills, proficiency, PPE or protective devices.

The employee continued to work until they realized arm was bleeding. The laceration sustained on left arm was glued back together. Employee was walking down the #4 mainline belt, and had stopped to look underneath the belt at 20blk. EE placed hand on the bed rail while looking under the belt, and coal fell out from the mine roof and landed on EE's right ring finger, causing a contusion on the finger and hand. Employee was complying with all laws, company policies and was wearing all appropriate PPE.

A rock fell while installing a bolt striking employee on the left side of the head, neck, shoulder area. Finished shift. Complained of pain & soreness. Was wearing required PPE and has training & experience in task.
