Miner was bolting the back with a jackleg. Loose material came out of the back, striking miner on top of head and right side of head, right shoulder and back. About 20 minutes later left rib came in, hitting the back of miner's legs knocking him down and partially buried both legs.

This miner was loading holes from the manbasket truck. A piece of loose came out of the back, striking him in the back of the head and pushing his chin into the railing on the basket truck. Cut to head & chin. Loose material hit back of head & upper back area. Right forearm hit railing.

This miner was changing steel. When putting the 8' steel in the hole, some loose hit the steel which flipped up hitting the right eyebrow.

WHILE PREPARING TO LOAD EXPLOSIVES IN A DRILLED HEADING ROUND A ROCK APPROX. 2' X 2' FELL OUT OF THE BACK FROM BETWEEN THE ROOF BOLTS. THE BOLTS HAD BEEN INSTALLED ON A 3' X 3' PATTERN. THE RO CK STRUCK THE INDIVIDUAL ON LOWER HALF OF HIS LEFT LEG. A SMALL PIECE HIT HIM ON THE NOSE AND LEFT CHEEK.
