Employee was working on utility lines at South Road 7 sump area when loose material fell from back striking employee. Employee was air lifted to the hospital and he was diagnosed with a fractured collar bone, crushed finger and broken rib.

Employee was jacklegging out of high scaler when he noticed a slab of loose from back. Loose fell out of back striking him on head and shoulders. He has taken to Salem ER and later transferred to Barnes in St. Louis. He has a fracture to L-2 vertebre and cut to right fore finger requiring sutures.

While in the elevated basket of an Elliot truck, two miners were hand scaling, while mining one piece of rock another piece broke loose and came down the bar and struck miner on the left arm, causing a serious laceration to the left wrist and forearm. Forearm required surgical repair.

AS EE WAS ASSISTING HIGH SCALING FROM GROVE MAN LIFT, A SECTION OF ROCK FELL ON THE BOOM OF THE RIG EJECTING HIM OUT OF THE BASKET. FALL PROTECTION WAS BEING WORN. THE EE DID SUSTAIN HEAD LACERATIONS REQUIRING 6 SUTURES, MUSCLE STRAIN TO LOWER BACK AND LEFT KNEE, AND CONTUSION TO LEFT THIGH. EE WAS RELEASED FOR WORK ON 5-30-03 BUT HAD SPENT ALL DAY AT THE DR. OFFICE ON 5-29-03

THE EE WAS ENTERING THE CAB OF A SCALER WHEN LOOSE FELL ON THE END OF THE EXTENDED BOOM. THE CAB INSTANLY "BOUNCED" TWO OR THREE TIMES, THROWING THE EE AGAINST THE ROOF OF THE CAB, AND THEN AP PARENTLY ONTO THE GROUND. THE EE WAS TREATED AT THE HOSPITAL, AND THEN ANOTHER HOSPITAL.
