While bolting the back a 6 inch rock came loose from the back and struck the miner on the hand on top of the drill causing a cut that needed 4 sutures.

EE was bolting back and when he went to collaring the hole a small piece of muck fell striking his left middle finger. EE's left middle finger sustained a laceration and he had to have stitches.

After blasting a round & barring down & then mucking out the round it was found that the lifters had not pulled. While cleaning out the holes to re-shoot them, a rock fell from the back or rib hitting the ground and rolling across the miner's foot breaking it.

While re-routing a 4" compressed air line a rock, measuring 1'x1'x8" being held in place and hidden from view by the air line, fell from 9' high & hit the miner on the right hand & foot. Cut on middle toe, 6 days later, was infected & miner was checked into hospital.

Worker was rock bolting, he was collaring hole for a bolt when he noticed his water was off, he shut the jackleg down and reached down with his right hand to turn the water on, when he looked away a rock fell out back hitting him on his left hand. Six (6) stiches were required to close the cut.

Worker had finished wetting down the muckpile, he had wrapped up the water hose and turned around to walk back into heading, when he was 5'-6' from water header he claims he was hit by a rock that fell out of back. Second opinion one week later required surgical repair, becoming reportable on 8/15/2007
