The employee's injury occurred when he was installing rockbolts with a jackleg. As the employee was drilling a bolt hole, a small piece of rock (approx 2" x 2") fell through the screened area of the back above the employee. The piece of rock struck the jackleg's rubber muffler then deflected striking the employee on his upper lip.

The employee stated his injury occurred as he was scaling loose rock from the back in N-60-D3. The employee states a piece of loose rock (size unknown) fell from back behind him, broke apart and a piece rolled down the muckpile and struck him on the back of his left leg.

THE EE'S INJURY OCCURRED AS HE WAS CONDUCTING BOOM REPAIRS TO THE ROCK BOLT JUMBO WHICH WAS SET UP AT A FACE. A SMALL PIECE OF ROCK FELL FROM THE BACK (THROUGH SCREEN) AND STRUCK THE EE ON HIS SHOULDER.
