A thin piece of rock fell from between the roof bolts and struck the employee's hard hat and slid down his neck. X-ray was negative; bruise and 4 stitches to neck. He returned to work his next scheduled shift, but missed the remainder of the week.

The employee was loading supplies on the roof bolt machine and a rock (3ft x 3ft x 3in)fell from between the bolts and struck the employee on his hard hat. X-ray and CAT scan showed a small crack to one vertebrae. Neck brace was applied.

The employee was carrying a post on his shoulder and a small rock fell from the top and struck his left little finger. X-ray showed a hairline crack in his little finger and 4 stitches were needed.

Ee was preparing to install a roof bolt in an out-by area of the 6 mains section. When the ee raised the canopy, a rock (3 ft x 1 ft x 3 inches thick) fell from the top & struck the edge of the canopy. The rock rolled around the edge of the canopy & struck the ee's lower back. The x-ray showed hair line fractures to 4 of the lumbar vertebraes. Dr stated that surgery isn't needed.

The foreman was walking away from the face and a rock approx 16" long, shaped like a wedge) fell from the xcut corner and the sharp edge struck his left leg below his knee. The xray was negative but 8 sutures were needed. The employee returned to work restricted duty his next shift.

The supervisor was using a pry bar to scale down loose top and when he pried a small rock a larger rock (18" x 18" x 18" thick) dislodged and struck the right side of his right foot. The x-ray was negative, but the laceration to his foot needed sutures. A staph infection required an additional night stay at the hospital.

WHILE STANDING BY THE ROOF BOLT MACHINE WITH HIS HANDS RESTING ON TOP OF THE MACHINE A ROCK (10 INCHES X 12 INCHES X 3 INCHES THICK) FELL FROM THE TOP AND STRUCK HIS LEFT HAND. X-RAY NEGATIVE, BRUISE AND 1 DAY RESTRICTED DUTY.

WIDE X 4 IN. THICK FELL WITHOUT WARNING AND STRUCK THE EE S RIGHT HAND. EX RAY NEGATIVE BUT 13 STITCHES WERE NEEDED.
