2LMN Unit Intersection 10+50 1R XC Actively bolting a brow on a blow through that was 4 to 5 feet in length. The brow broke up causing multiple pieces of shale to fall. Employee was inserting pin into previously drilled hole when employee was struck on the back and legs knocking employee to the ground.

While roof bolting, EE's drill steel became clogged. As EE reached to remove the steel, shale fell from the roof hitting EE's hand and driving EE's thumb into the drill pot. The impact caused a contusion to EE's right thumbnail. After shift EE went to an urgent care facility where X-rays revealed a fracture to the tip of EE's thumb.

While roof bolting, reached to remove a plugged drill steel from the roof. As they reached, shale and rock fell around the steel, knocking the injured to the ground. The rock and drill steel struck their right calf and left a laceration that required sutures to close.

While roof bolting and removing a steel from the roof, employee was lining the steel up with the drill pot when a piece of shale struck employee's hand mashing it against the pot. The impact caused a fracture and laceration to the right index finger.

He initially returned to work without restriction but a follow up visit scheduled with a surgeon advised installing pins in the joint placing him on a limited work restriction 10/6/2008.

While operating a continuous miner, a piece of shale slid off the miner and struck the operator on the left foot. Transported by private vehicle to St. John's Hospital Springfield where an X-ray revealed a fracture to his big toe. Released with minimal restrictions and did not miss any work.
