When employee went to change his drill steel out, a piece of draw rock fell hitting him on the right wrist. He had a simple fracture to the wrist.

Employee was walking from #6 to #7 entry to get the scoop when a piece of draw rock fell between the rib and roof bolt striking him. It knocked him down and landed on his foot.
Section Foreman was sitting on the rear bumper of the Roof Bolting machine when a piece of draw rock fell striking him in the top of his head. The rock measured 12 inches by 16 inches by 1 inch thick, mining height in this location measured 7 1/2 ft. The accident occurred in #3 entry last open cross-cut on the #5 working section.
EE was bolting in left X-cut as he went to pull up for the next roll of bolts a piece of draw rock fell out between the rolls of bolts hitting him on the right side of the head, shoulder and arm. He received a cut to the right arm that took 20 stitches.
Employee was walking back to the roof bolter in 2 left X-cut and a small piece of draw rock fell from top and hit him on top of the left hand cutting his hand.
EE was making a water line splice on the miner. He was down on his knee and a piece of draw rock fell from the top hitting him on the back of the head and cutting him on the head. EE received 5 staples in the back of his head.

Employee was nailing a piece of curtain to the rib and a piece of draw rock fell from the top hitting him on the left forearm puting a 3" cut in it. He received 4 stitches.

Apiece of rock fell out of the top between two rolls of bolts hitting him on the left shoulder and top of left hand cutting it. The rock was about 1 foot by 3 foot and 0 - 4 inches thick.
The employee was bolting in the #10 heading when a piece of rock fell out from the top hitting him in the right arm. The rock was about 18" around and 1.5" thick. He received a cut to his upper right arm that took 8 stitches.
Employee was rolling rock dust hose up onto the duster and pieces of draw rock about 8" circle and o to 1" thick, fell hitting EE on the top of the little finger on the right hand, cutting it about 1 1/4" long. (4 stitches)
The employee was building a stopping in #7 entry when a piece of rock fell from the top hitting him in the right side of head and shoulder. It caused him to fall on a block. When he landed he landed on the block and it cracked his sternum. The rock was about a 2' x 2' x 2" thick.
After bolting 5 left x-cut ee walk to the back of bolter and was talking to the section foreman then as he was walking back to the front of bolter a rock fell out an hitting him on the back an dleft leg. The size of the rock was 4 foot by 3 foot by 3 inch thick.
While EE was turning a right X-cut, a piece of rock broke loose from the top and swung back after being hung on a pizza pan and falling, striking EE on the right side of the body.
The accident occurred on #002 section when a kettle bottom fell from top striking the employee on Rt side adn sliding down employee. Resulted in a broken bone on Rt lower leg.
Employee was bolting top. Rock fell in front of ATRS. EE started to run striking head on personal canopy. He worked up until 3/23/07 & then started missing work.

EMPLOYEE WAS SETTING UP CANOPY AT A ROOF FALL; HE WAS PUSHING ON ARCHWAY TO STAND IN TRACK & ARCHWAY HIT TOP & JARRED PIECE OF SLATE LOOSE WHICH HIT HIM IN THE BACK OF HIS NECK.

The employee was bolting in 6 left x-cut. He was putting up his inside bolt in the 3rd row when a roof rock broke loose in between canopy and T-bar. The rock broke when it hit the canopy and part of the rock fell hitting him in the back and legs
