Incident found in routine audit - Employee was holding the alimak rail so it could be bolted together. A rock fell out of the back striking him behind his leg.
While installing resin cartridges in a bore hole, a slab of rock came loose and struck the employee in the nose. The miners were working from a supported area trying to tie the mats and bolts into the face and back.
