While, inserting drill steel in the pot to install extra bolts a piece, of rock fell measuring approximately 20''x 12''x 5 inches thick striking the employee left hand causing a fracture to the base of 4th distal phalanx.
EE was roof bolting in the #6 entry on #1 West Mains Section. While, bolting EE noticed some draw, rock so EE proceeded to scale the mine roof with a slate bar. As EE was scaling, a piece of rock fell behind EE, striking EE's left leg causing a broken, Tibia and Fibia.
