Employee was cutting #6 entry when a rock measuring 12" wide X 30" long X 1/4" to 1.5" thick broke from the top striking employee on the head and left shoulder area. Employee said neck and shoulders were hurting so after injuries were assessed employee was taken outside and transported to hospitals.

Showing all 3EE went to move the 4 man diesel, pulled it up about 8-10 ft. to get it out of the way. EE said that a piece of rock fell and hit them in the head and left shoulder. This happened between 2:30 and 3:30am and they continued to work until around 4:oo when they were transported outside. Also MSHA was called,because the ambulance EMT called life-flight. EE was sent to St. Vincent.
While employee was scaling rock in #2 entry on #1 Unit he moved a smaller rock dislodging a larger rock striking him on the top of the head pushing him toward the return side rib.
