Employee was shoveling belt at Brk 7 on the #4 Belt when a piece of draw rock struck them on the top of the head. Employee was taken off work on 11/16/2018 and has not returned to work.
Employee was waiting on a coal Hauler to return to the miner and had backed the miner up in supported top to get a snack and drink from their dinner bucket that was on top of the miner. When employee was stepping away from the miner a section of draw rock fell out striking them in the back and lower left leg.
Employee stated was bolting in 1 left, was drilling with bolt machine when rock fell between bolts and ATRS hitting the employee in the right shoulder and arm. This occurred inby spad 434.

INJURED HAD PUT UP EE'S SECOND BOLT IN THAT ROW AND NOTICED THAT IT WAS WIDE TO THE RIB. EE SWUNG THE BOOM OVER TO INSTALL AN EXTRA BOLT. EE STARTED DRILLING AND A SECTION OF ROOF FELL ON EE. STRIKING EE ON HEAD AND SHOULDERS.
INJURED WAS TRAVELING TO SECTION ON A LIL MAC THREE WHEELER WHEN A PIECE OF DRAW ROCK FELL STRIKING EE ON THE UPPER PART OF EE'S BODY. EE SUFFERED A LACERATION BESIDE EE'S LEFT EYE, REQUIRING 8 STITCHES. A CT\SCAN WAS DONE AND FOUND NO OTHER INJURIES.
Injured was tramming a scoop underground from surface. At break 38 a piece of rock fell from the top striking injured on left shoulder. X-rays revealed no broken bones or dislocations.
Injured was bolting top when a piece of rock fell from the top, striking him in his upper body, pushing him to the ground. He was setting up to install the outside bolt. After emergency room visit, was reported that nothing was broken, but he would be sore.
