Injured employee was installing roof bolts from the right side of the continuous miner while slabbing part of a belt trench. As EE started drilling a bolt hole, a piece of rock approximately 2 inches thick by 16 inches in diameter fell and struck EE's right shoulder. Employee stated that the roof appeared good and EE had installed 7-8 bolts without any issue.

EE was installing roof bolts in an overcast brow when a rock fell striking the right side of EE's head/shoulder causing EE to fall to the mine floor. Piece of rock also struck EE's left ankle. EE was treated for a left ankle contusion and given a brace. EE didn't violate any rules or regulations and was wearing required PPE. Returned to work EE's next shift.

Injured employee was installing roof bolts from the left side of the continuous miner while mining in the #4 entry of the 1W CM section. Mining through a clay vein. A rock fell while installing a bolt striking employee on the left side of the head, neck, shoulder area. Finished shift. Complained of pain & soreness. Was wearing required PPE and has training & experience in task.

EE was the roof bolter on the left side of the CM on the 1-W Section, operating in the #2 entry, 100 blk. EE just completed EE's test bolt/test hole procedures. EE turned and started walking outby to the ATRS controls when a piece of rock/coal fell striking EE on the head. EE got sick and had head/neck pain. EE didn't violate any regulations and has training and experience.

EE was installing roof bolts at the face when a piece of rock was dislodged and fell striking EE on the head causing a laceration above the left eye. EE received 3 stitches. The piece of rock was approx. 8"L x 4"W x 1" Thick. EE was wearing required PPE, has training & experience. No rules or regulations were violated.

EE was standing beside a single head roof bolting machine when 2 small rocks "popped" out from the mine roof and fell. 1st rock knocked off the EE's hard hat and the 2nd rock struck the EE on the head causing a laceration approx. 1 1/2" in length. 5 sutures were required. EE has experience and is properly trained. Was wearing all required PPE.

Employee was installing roof bolts at the roof bolter station on the left side of the continuous miner. As EE finished installation of a roof channel, a rock fell between the channel straps striking the employee on right shoulder causing a contusion and abrasion. Employee has training & experience and was wearing required PPE.

EE was roof bolting from the continuous miner. A piece of "head" coal fell from the roof and struck EE on the head, knocking EE to the mine floor. Witness stated EE was unconscious for 4-5 seconds. Was diagnosed with a concussion and separated right shoulder. EE did not violate any rules, regulations & is trained and experienced. Was wearing required PPE.

Employee was taking a ventilation tube to the face of the CM section when head coal fell out hitting employee on the head. EE's hard hat was knocked off and a smaller piece hit the top of EE's head causing a laceration. EE was later diagnosed with a concussion & began missing work. No rules or regulations were violated and employee is properly trained. Was wearing required PPE.

While handling a cable spool, a piece of roof rock fell from between the roof straps striking the employee on the side of the right hand. The accident happened when the employee wasn't paying attention to the conditions around the employee. The employee was properly trained and did not violate any rules, regulations or policies.
