Employee claims to have been hit on the head/right ear by a rock as employee was walking down the longwall face near #1 shield. Employee complained of dizziness & suffered a small laceration above right ear that did not require sutures & employee never lost consciousness. Employee was working near the crusher area of the 7 West Longwall, when a rock fell from the top, striking EE's right shoulder causing a contusion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was struck by a rock while putting together drill steels before beginning to bolt off of the twin boom bolter. The rock fell from the roof and hit the ATRS canopy before striking the employee on the chin, causing a laceration that resulted in four stitches. The individual was hanging a bag when a rock fell from the roof and knocked hat off. When they reached down to pick up the hat, another small rock fell from the roof striking them in the head. (7) staples.

EE was observing the installation of roof bolts with Fletcher CB on the 4WTG section. A rock approx 4'x6'x12' fell striking ATRS then ee on the lt lower leg resulting in a comp fracture. On 10/8/17 the injury resulted in amputation of the leg at the knee. Cause of injury was awareness. Compliance with rules/regs,PPE,mining equip or Miner proficiency not a factor at this time
Employee was walking to the loader in the 8 South Section 7-8xc. Employee stepped in a crosscut for a shuttle car to come by when a corner fell out striking employee in the legs resulting in contusions to both legs. Miner proficiency, skills or attitude was not an issue. PPE was not an issue.
Employee was helping install roof bolt with a gopher drill at 7 1/4xc in 4WHG when a rock fell striking left hand causing a sprain and laceration. Employee was cleaning a rock off the top of a Miner in the 4 West TG section when a rock fell from the top left side of roof mesh striking EE's left hand resulting in a contusion. Employee was in the process of getting rocks out of throat of miner in the 4WTG section when a rock fell from in between the straps striking right hand causing a laceration and fracture.Cause of injury was awareness. Employee was helping center bolt in the 4WTG section under the bolter canopy when a rock fell slid down the steel and struck employee in the face causing a laceration which resulted in sutures from plastic surgery. Employee was installing rib bolts on CM miner in the 4West Tailgate section and reached for a bit a rock fell striking right hand causing a fracture to index finger requiring plastic surgery. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies
Employee was bolting on left side of miner in the 4WHG section when EE reached for pusher steel a rock fell from between straps striking left hand causing a laceration between EE's thumb and finger resulting in two sutures. EE returned on 3-15-17 EE's next scheduled shift. No other part 50 criteria applies
