Hogg Contractor bent over to pick up hand tools. After EE stood up, a piece of draw rock dislodged from the roof falling striking EE on EE's right foot. This resulted in a fracture to the #3 and #4 metatarsals.
Employee had just installed a cable bolt and was straightening up the tray when a piece of draw rock fell out between the coal rib and outside bolt striking EE on EE's lower back and right leg. This resulted in a fracture to EE's femur.
Employee was installing first bolt on the second row of bolts in the 4L punch through. EE had drilled the first piece of steel and connected the second piece of steel. While drilling the second piece of steel, a piece of draw rock fell and hit the drill pot then struck the employee in the mouth resulting in 3 stitches.
Employee had loaded roof glue into the tray of the roof bolter and stepped down into the bolt station when a piece of draw rock fell onto the canopy and part of the rock hit EE on the left calf. EE did not receive medical treatment at the ER other than cleaning and a Bandage. EE began days away from work on 6/24/24 due to a pocket of infection in EE's calf.
Employee was making up a bolt for a red hat. When EE went to hand the bolt to the red hat, EE placed hand on the top of the fan and a piece of draw rock fell striking EE on top of the left hand. EE continued to work until 4/26/24.
Employee was standing in the walkway of the roof bolter preparing to bolt in the #2 heading. A rock 69" x 21" x 0-8" thick fell from the roof and struck EE. EE received lacerations to the left forearm, right hand, and head.
Contractor was in the process of hanging curtain with a pogo stick when an undetected piece of draw rock fell from the mine roof striking them resulting in a fracture to their back.
Employee was in the process of scaling draw rock in the intersection of the #4 entry when an undetected piece of draw rock dislodged from above a piece of wire mesh and fell striking the employee on the head, right hand and right leg. This resulted in stitches/staples to the hand and head and a fracture to the leg.
Employee had installed inside bolt of second row and was in the process of drilling the hole for EE's middle bolt. EE's steel had stopped up and EE removed the bottom piece to clear it out. EE re-installed EE's steel and was pushing it up to finish drilling when a rock struck EE on top of this left forearm resulting in a laceration and sutures.
Employee bent over to pick up a pogo stick laying on the ground near the installed vent curtain. A piece of draw rock dislodged from the mine roof with height of 8'2" hitting employee's right hand resulting in amputation to the right thumb.
Employee was in the process of turning a left hand x-cut on the second lift when a piece of rock slid down out from the flush face, hitting the ground and flipping over striking the employee and knocking EE to the ground. The employee landed on material on the mine floor and sustained a non-displaced fracture in the upper part of the right fibula.
Employee was making a shuttle car splice when a rock fell out of the top striking ee on the shoulder, hand and leg. This resulted in lacerations to fingers and a fracture to 5th metacarpal.
Employee was drilling a hole for a roof bolt when an undetected piece of rock fell striking employee's lip resulting in a laceration and a chipped tooth.
Employee was standing beside of drill when a piece of draw rock fell out between bolts striking EE on top of the hard hat. This resulted in a sprain to EE's neck area.
While employee was installing a 5' roof bolt after drilling the hole, a piece of draw rock fell, hit the pan on the bolter, then fell and hit employee on the left foot. This resulted in a cut to the left small toe that required stitches.
Employee was loading shuttle car when ee completed the car ee turned to back the miner up to set over and a piece of draw rock fell and struck right hand resulting in a fracture. Employee continued to work up until 11/9/2017.
Employee was walking from the #4 entry to the #3 entry when a piece of rock fell striking employee's left arm causing a laceration resulting in stitches.
Employee put 1st bolt up, swung over 13" to drill test hole, drilled 3' starter, was taking starter out of chuck after drilling. Rock came loose sliding down and striking employee on index finger cutting it resulting in a laceration requiring stitches.
Employee was changing bit when an undetected piece of material fell, sliding down from the canopy striking employee in lower back resulting in a bruise.
Employee was bolting the 2nd cut out of 4R. While installing a rib bolt in 3rd row of bolts a piece of draw rock measuring 70.5" long, 0" to 6.5" thick and 46.5" wide fell out from the top and the edge of the rock caught the back of EE's right heel causing an avulsion. **Also fracture to heal bone.**
Employee was putting up a rope bolt when undetected material slid from the canopy striking ee on the right shoulder.
