The Roof Bolter was advancing up to install the next row of bolts, two pieces of rock fell between rib and installed roof bolt, first rock struck them in head knocking them to mine floor second struck them in the right knee causing contusions and swelling.

Showing all 5Employee was working on miner in face when a rock fell on ee back pushing ee down into the 14/15 Joy miner, fracturing both collar bones and ribs on both sides. EE was hospitalized and received surgery.
Operating miner when a large piece of draw rock fell from mine roof pinning employee to the floor. Resulting in fractures to back, nose and ribs. Lacerations to face and lip. Was hospitalized and later released.
Employee was bolting roof when a rock fell from roof on rib side and struck right foot. Employee was taken to hospital and had a contusion on foot.
Employee was replacing a panel on a shuttle car when a piece of draw rock approx. 23" X 8' long and 2 1/2 to 5" thick fell between bolts and slid down EE's back before striking EE's left foot. Foot wasn't broken but EE was hospitalized in order to receive antibiotics through an IV. EE was released on 03/26/2017. EE returned to work on 03/27/2017.
