Employee was standing beside the continuous miner waiting to connect and hang the next ventilation tube. While they were waiting the top fell out between the roof straps and struck them knocking off their hard hat. A small piece of coal/rock followed and struck the top of their head causing a small cut. They received one staple to close the cut. Returned to work their next shift.

Injured employee was assisting a co-worker hang a ventilation tube from the mine roof at the face beside the continuous miner. As they were hanging the tube in place, a rock fell from above the tube and struck employee on EE's right shoulder and upper leg / thigh area. Has swelling and bruising to EE's leg. No rules/regulations were violated and employee has training & experience.

Injured employee was roof bolting from the continuous miner at the face on 8W CM Section. After installing a bolt, employee walked back to the bolt tray to get another bolt and a piece of coal fell from the roof and struck EE on the right shoulder. Employee stated roof conditions were good and coal fell without warning. Has pain & soreness and possible Labrum damage.

Injured employee was walking up the #3 entry toward the continuous miner when a piece of coal 4-6" thick fell from between roof straps and struck employee on the back of head/neck/shoulders. Has pain and soreness and is off work awaiting an MRI. X-rays were negative. Normal roof conditions at accident location.

Employee was replacing the longwall headgate drive winch. As EE was removing bolts from the winch, a piece of coal/rock fell out from between the roof channel straps and struck the back of EE's leg forcing EE against the headgate drive. EE has pain and swelling in the right knee and ankle. No rules or regulations were violated.

Injured employee was standing near the right side roof bolter station of the CM. CM was being serviced and injured employee handed a bucket of bit inserts to another employee and a small piece of coal/rock fell and struck the top of EE's right hand causing a laceration. Employee received 11 stitches to close the cut. Returned to work the next shift.

Injured employee was installing roof bolts from a center bolter. Placed right hand on the drill guide and grabbed a drill steel with EE's left. While reaching for the drill steel a rock fell and struck employee's hand on the drill guide. EE was diagnosed with a fractured middle finger and 14 stitches.

Injured Employee was installing roof bolts from the left side of the CM. Stated knelt down on left knee. As EE was kneeling, a piece of coal fell from the roof and hit EE's right knee causing a cut & contusion. Employee received 3 sutures to close the cut. No rules or regulations were violated & employee has training & experience.

Employee was operating the Roof Bolter installing roof bolts on the 6W Longwall Development face. As employee was installing a bolt, a large piece of rock/coal fell out from the roof onto the ATRS and broke up. A piece of rock struck employee in employee's back, knocking EE forward causing EE to twist as EE fell down. Employee has pain in lower back and left knee.

Employee was roof bolting on the left side of the continuous miner. As EE was drilling a hole for bolt installation, a rock fell out & struck the machine mounted rock deflector & broke into smaller pieces. One of the smaller pieces then hit the rib protector & then struck employee in the back of their lower left leg. No rules or regulations were violated & employee has training.

EE finished a bolt installation and reached back to turn on the "tram" switch when a 12"x12"x3" piece of head coal fell out between roof straps and struck EE's upper right forearm causing a contusion, pain & swelling. EE didn't violate any rules or regulations and has been Task Trained.

Injured employee was setting in the dinner hole outby the 7 North Mains CM Section eating lunch. A piece of head coal fell from between roof channels and struck their hard hat, knocking it off. As they were reaching for the hat another piece of coal fell and struck their head causing small cuts/abrasions and one cut requiring 1 suture.

Injured employee was operating a CM as part of the "mucking" process at the Raised Bore Intake Shaft drilling site. EE was standing at the rear of the CM when a rock fell from the shaft and ricocheted off of something and struck EE on the right forearm. EE's right radius bone was broken as a result. Employee was wearing required PPE & didn't violate any rules or regulations.

Employee was standing in the last open cross-cut near the headgate area of the 3W Longwall, waiting for the face to advance before EE entered the area. As EE was standing there, a piece of coal fell from the roof and struck EE on the hard hat and between EE's shoulder blades. Employee began missing work on 7-21-21 due to this injury. EE didn't violate any rules or regulations.

Employee was roof bolting on the right side of the CM when a large piece of "head coal" fell out from 2 roof channel straps and struck EE on the head, neck, shoulder. Employee was briefly knocked unconscious. Was transported to the hospital for evaluation and released later that shift. Employee was wearing required PPE and has training & experience.
