Injured installing rib bolt while cutting turn 3-2 at 115 block 9 North when a piece of the rock brow fell out and struck EE's left shin resulting in a laceration that required 9 sutures to close. No problems with equipment or compliance issues, training or PPE.
Injured was bolting off side of miner in St Leo 113 blk 3-2 with new section of roof mesh, not bolted to top yet, on the miner when a piece of coal (3'x3'x5") dislodged, slid down the screen, bounced off the rib and struck EE, resulting in a fracture to the L1 vertebrae. No problems with equipment or compliance issues, training or PPE.
A small piece of rock dislodged and struck employee in the hard hat. Employee was taken to a hospital to be evaluated as a precautionary measure and was excused for the following shift. Injury was not the result of a training or compliance issue.
Injured was on right side of miner on 6W section installing a ventilation tube when some roof material (size unknown) dislodged and struck EE's hard hat, causing EE to fall down. No issues with compliance, equipment, job skills, proficiency, training or PPE.
The EE was bolting on the miner in the 9 North Mains Section. As EE was inserting glue in the hole for a bolt, a piece of roof material fell striking EE in the lower back and right lower leg. The injured finished the shift, but began missing work the next shift 12/12/23.
The EE was rib bolting in 9 North Mains on the operator's side. A piece of roof material fell out and rolled off the bolter rock deflector and contacted the injured EE's left foot and leg. EE sustained hairline fractures in EE's foot, and began missing work 11/9/23.
The EE was working on the 5W Section. EE was back by the FCT Hopper, when a 6"x6"x3" piece of the mine roof fell striking EE on the head and face. This caused a laceration to EE's lip. The EE received 6 stitches, and did not miss any scheduled shifts.
The injured EE was tubing on the 5W Section. The injured was standing back by the FCT, when a piece of head coal and rock fell from the roof and struck the EE on top of the head. The piece was approximately 2'x2'x5" thick. The EE briefly lost consciousness. EE was treated and released from the hospital on 9/11/23. Began missing 9/12/23.
The injured EE was driving down MW Haulage. Between 50-60 block, the mine roof sloughed out. Some of this material contacted the EE's face and a piece was stuck in EE's left eye. EE did not finish the shift and went to seek treatment. The injured was prescribed eye drops. The EE didn't miss any time.
The injured EE was bolting on the miner in the 5W Section. A piece of the mine roof fell between the rock deflector and rib door striking the EE in the lower back. EE sustained only bruising, but began missing work 7/18/2023.
Injured was installing a roof bolt in #2 entry of the 5W CM section when a chunk of rock binder fell out and hit EE's left foot causing a contusion. No problems with equipment or compliance issues, training or PPE.
The EE was walking around the curtain of #6 seal of the 3 North Rehab, when a rock fell from the mine roof and struck the EE's left shoulder. The injured finished the shift, and began missing work 5/3/23.
The EE was helping load supplies on the miner. While on the right side of the machine, laid their hand on the rib door. A piece of material fell from the mine roof and struck EE's right middle finger causing a fracture and laceration. The EE was wearing the appropriate PPE. EE began missing on 4/20/23. Also received 5 stitches.
The EE was bolting on the right side of the miner in 4 West Section. As the EE was drilling a hole in the mine roof, a rock fell out striking EE's left arm. This caused a laceration and EE received 5 stitches. EE did not miss any time due to this injury.
While walking on Sugar Run Bottom tail track a rock dislodged from the top and fell on the injured employee's left shoulder causing a contusion. No problems with equipment or compliance issues, training or PPE.
EE was bolting on the section, when a piece of slate fell from the mine roof and struck the EE's left arm causing a laceration. EE received 15 stitches, and did not miss any work.
The employee was rib bolting on the return side of the 4 West Miner. When a piece of rock approximately 16"L x 4" thick, fell from behind the tube striking the EE in the head and neck area. EE did not finish EE's shift and was taken for further evaluation. The EE was trained in the task.
Injured was hanging a ventilation tube on the 3 West section at the face by the miner when a piece of the rock binder fell out and struck the employee in the right side of the head. No problems with equipment or compliance issues, training or PPE.
The EE was bolting on the miner in the 2 West Section, 1-2 x-cut at 55 block. As the EE went to put drill steel in the chuck, a piece of roof rock fell out and struck right forearm causing a laceration. EE finished shift and sought treatment after. Received 6 stitches, missed no time.
