Miner was bolting when loose material fell from the back causing a laceration on the right forearm. The laceration required sutures.

While barring down in the 49-173 heading, loose rock from the back fell out and struck the miner causing lacerations and contusions to the neck, shoulder and left elbow. They received sutures and was placed off work by provider.

While bolting the miner was holding the end of a 6' mat in place while EE's partner was driving the bolt. Rock fell out of the wall and pushed the mat into EE's face causing a laceration on EE's face, EE then fell back and the rock struck EE's lower leg breaking EE's ankle.

Drilling holes in the back with a jackleg; a piece of rock fell out of the back and pinched off the end of left thumb.

Climbing to top of raise, rocks fell out of the back, Miner ducked back into the raise and a rock lacerated Miner's left forearm.

The injured Miner was in basket of the Alimack raise climber, with a partner. The Partner was barring down. Miners hand was on the rail. A rock came down and hit the miners left hand. Injury required surgery.

Nipper was assisting with loading the round. The nipper's hand was resting on the prell pot. A small rock came down and cut left wrist.

Miner was cut on forearm by a piece of quartzite.

Miner was bolting the back in 4900-181 stope. A small rock fell and cut miner's chin. Laceration to chin required sutures.

3200-033 miner drilling round in the 033 raise, rock came out of crown landing on right elbow.

5500-120 collaring hole with jack leg drill, rock came out of back hitting right knee. 1-1/2" laceration.

5200-159ER scaling back under protected ground, rock came out of back hitting miner on left shoulder

4900-334 bolting back - rock came out of back hitting miner on upper lip

4900-310 Load-out barring back down after blast, stepped back to look for better angle to bar; material from behind wire fell out hit miner on the shoulder.

3400-049E prep and scale for rock bolting in raise stope, miner bolting back, material came out of crown of hanging wall hit left shoulder.

Alimak Raise, bolting after blasting round - just got collared when rock size of baseball came down hitting between glove and arm guard.

4900-360 Collaring hole to drill for roof bolt, material came out of back hitting bridge of nose.

Miner bar down loose material during work place exam. Loose material came out of back, bounced off vent bag, hit miner in mouth.

The employee was collaring a hole with a jackleg drill for a slab round when a small piece of rock broke off at the collar and struck him on the shin. He received a laceration which required sutures.

Employee was bolting up a round that had been previously blasted. He was in the process of tightening a dywidag bolt when a rock directly over his head fell and contacted the front of his hard hat. This caused what was diagnosed as a neck sprain.

Employee was barring back when a small rock slipped from behind bolt plate and hit him on the left wrist causing a laceration requiring sutures.

Employee was rock bolting the back when he encountered a rock that had a crack in it but was too tight to bar down. He decided to moil it down with his drill but when it broke loose it contacted the footwall and deflected toward him. It struck him on the right calf causing a laceration which required sutures.

Employee was barring down when a small burst occurred which dislodged a rock, striking the employee on the hand. He was wearing all PPE including gloves.

49-172E EE was collaring hole in back when piece of rock approx. 3" x 2" x 1" broke off, fell and struck him on "L" wrist between glove and arm guard. Stope was barred down very well.

Barring down loose material prior to bolting back in the 4300-275. Material came down and hit employee on right wrist (between arm guard and glove) causing laceration.

4600-165 - Rock bolting, rock came out of back hitting miner - right wrist.

Drilling and bolting with jackleg drill, miner had installed approximately 8 bolts when he started to drill the next hole. As he was drilling a rock loosened from drilling, falling and striking "L" thumb. Employee was wearing gloves and long sleeve shirt at time of accident.

Miner was in process of bolting, drilled for bolt, noticed loose area, left machine and steel in last hole drilled and barred down loose material. Loose came out of hanging and hit machine, then came down and hit employee on "L" thigh. Loose measured 12" x 8" x 4". Bruise to "L" Thigh.

Employee barring down, a rock that was being barred down came out of the back and struck employee on "R" elbow.

4600-146E Ramp. On June 4th EE was rock bolting behind brow, rock came from behind brow down rock bolt steel hitting "L" hand. EE went back to clinic on June 24th for follow-up. X-rayed again. Fracture to "L" distal radius, cast was applied

EE started shift by barring down his stope 210S from raise to face. Rock came out from behind roof bolt and holy board, hitting EE on "R" Shoulder.

EE was reaching for his jackleg to take to face, when he was struck on top of head by unknown object (EE had hardhat on at time of accident). There were two basketball sized rocks in walkway, probably was hit by one or both of them.

Bolting in 4600 Vent Raise rock came from behind ladderway and hit EE behind "L" knee

EE was lacing the outside of the raise with lagging. He raised a piece of lagging to put in place and the lagging came in contact with the back, this caused a rock to fall out of the back striking the EE on the back of the neck causing the laceration.

Employee was installing ground support. As he was tightening a rockbolt, a rock fell striking him on the upper leg causing a laceration requiring sutures.

Employee was installing ground support. As he reached to change his steel, a rock struck him on the hand, pinching his little finger between the jack leg drill and the rock, causing a laceration to his finger which required sutures.

Employee was barring down in a drift heading onto previously barred material. A rock deflected and struck the employee in lower leg resulting in a laceration requiring sutures.

The employee was rockbolting the back when a rock fell catching the employees left hand between the jackleg and the falling rock, lacerating and fracturing his index finger.

Employee was bolting the back in the stope when a rock fell striking the employee on the left shoulder causing a laceration requiring 3 sutures.

Employee was rock bolting and was struck in the mouth, causing a laceration, requiring sutures in his lip.

Miner was drilling with a stoper when a small rock fell causing a laceration on right wrist requiring sutures.

Employee was rockbolting. A rock fell from the back cutting the employee's left wrist.

Rock bolting the back in the drift, employee saw a rock fall from the back. In his attempt to step back out of the way, the rock hit his left shin and foot.

Employee was rebolting the drift back. Piece of rock came down hitting employee between his glove and arm guard causing a wound that required stitches.

EE was rock bolting the roof when a piece of rock fell and struck him on upper right arm. Rock size approx. 2" x 4".

WHILE THE EMPLOYEE WAS ROCK BOLTING A SMALL ROCK BURST OCCURED IN THE BACK CAUSING A SMALL ROCK TO STRIKE HIM

The employee was in the process of rockbolting the back in a stop when a rock came down and struck him on the arm causing a small laceration.
