Employee was bolting top in #4 entry when a piece of draw rock fell striking him in the left shoulder. The rock measured approximately 3' X 2' X 1" to 10".

While bolting top the roof bolt operator turned to make a bolt up and a piece of roof fell striking the employee on the right ear causing a laceration, Employee received first aid at scene and was transported to HRMC at Prestonsburg Ky,where he received stitches to his right ear.

Employee was bolting a place that had fallen out. He had installed 2 rows of bolts and was on the 3rd row inside bolt when a rock fell striking canopy, then striking employee on right ear causing a laceration to the right ear. He was taken to local hospital where 3 stitches were used to close the laceration.

EE WAS STANDING BESIDE THE FLETCHER ROOF BOLTING MACHINE IN THE LAST OPEN CROSSCUT. A PIECE OF DRAW SLATE (3' WIDE BY 5' LONG BY 2' THICK) FELL STRIKING HIM ON THE NECK AND RIGHT SHOULDER KNOC KING HIM TO THE MINE FLOOR. HIS INJURIES WERE CONTUSIONS TO THE NECK, SHOULDER AND RIB CAGE. HE WORKED HIS NEXT TWO SHIFTS BUT WAS UNABLE TO WORK ON 5-14-01.
