While installing the inside bolt in the first row in the Number 7 entry the employee had set the starter steel and as he was removing his hand a rock fell from the top landing on his hand smashing it in the Drill Pod resulting in a laceration and ten (10) sutures to his right index finger.

Employee was traveling along the track on 4belt. When a piece of rock (measuring 4'X 2.5' X4") Fell from between the bolts and struck the Employee in the right shoulder knocking him off the ride. Injured went hospital and received a Diagnosis of Injury to the head, sprain of shoulder, contusion and a Concussion.

Employee was troubleshooting the RCM when a piece of draw rock fell striking Employee in the head. He was treated at Beckley Appalachian Regional Hospital where he received nine (9) staples in his head.

While bolting a piece of rock fell and smashed Employee's left middle finger against the pot. Employee was treated at Raleigh General Hospital where he received seven (7) sutures.

While Employee was standing in #4 entry top brow fell off striking Employee in both legs. Employee was transported by EMS to CAMC General Division. (Resulted in fracture of lower right leg.)

Employee was bolting on MMU003 in #1 entry, spad #2770, when a 28"x4" piece of draw rock fell from the roof onto his right hand. The height was about 6 1/2'. He received a fracture to his hand and was placed on restricted duty.

While roof bolting in the last row in the face of #3 entry, employee bent a bolt. While drilling a new hole a piece of rock that came loose on the bent bolt broke at his right forearm. He received a small contusion without breaking the skin. Doctors gave him two days of restricted duty. He returned to work on 11/11/2011.

Roof bolter operator had finished setting two 6' torque tension bolts & waited for his offside partner to finish his, when a rock 3'x 3'x 2" thick fell from between bolts & knocked his hard hat off, hitting his right hand. A second piece of rock fell and lacerated the back of his head. (the roof is mixed & 7.5 feet.) The laceration required 8 staples. #2 Section, 4R, inby spad 2579

Operator was bolting in 5R on #2 section using 12' cable bolts when a rock measuring 21" X 10" fell from between bolts striking his left shoulder. He received a laceration that required two stitches. The height was 11' and the top mixed. He missed no work.

Electrician was replacing a battery plug when a rock measuring 5'x3'x3 1/2" fell from the roof striking him on the back of head & neck area. He drove himself to Raleigh General and the doctors gave him a full release. He returned to work & continued to work until 9/27/2011. He went to his doctor who gave him two days off with muscle relaxers. He returned to full duty on 9/30/2011.

EE was bolting #6 right on 002 MMU when a piece of draw rock measuring 18' x 8' wide fell and struck his right forearm. He was not wearing the Kevlar arm sleeves provided. He received a laceration. He refused ambulance care and drove himself to the VA hospital, where he received seven sutures.

The miner operator was backing the machine out to set over when a piece of rock approximately 4' X 4 1/2' X 3/4 to 3" thick fell from between bolts onto the miner and broke apart. The rock slid off the machine striking his left knee area. Doctors place him on restricted duty for two working days.

EE was bolting in #1 entry when a small piece of roof fell, striking his left index finger. He received a laceration and ceased work. He drove himself to the hospital and received five stitches. He returned to work on his next shift.

Employee was bolting #5 left crosscut on #2 section when a piece of draw rock (1' x 2' x 1") fell onto the operator's side tray. It struck his left forearm and left a 4" laceration. He was treated and sent to the Raleigh Boone Clinic where he received six stitches. He returned to work and continued bolting for the remainder of his shift.

Operator was bolting #3 entry on #2 section when small pieces of rock fell from the top, striking his right arm. He ceased work and drove himself to the hospital where he received three stitches for a small laceration to his arm. He returned on his next shift.

Employee was walking around the mantrip which was parked at the end of the track in a spur for the new section. He was going for electrical tape. He heard a flyboard crack and then rock knocked his hardhat off. The rock hit his left shoulder and then rolled down to the back of his leg. His leg and foot were bruised. He was instructed to keep it elevated for three days.

EE knelt down by the pressure pump to fill out a report. He removed his gloves and laid his left hand on the pressure pump cage as he knelt. His head bumped a pizza pan and dislodged a rock measuring 14"X12"X1. It fell and cut three outside fingers. He finished the shift. Clinic did not treat the cut except for antiseptic until an orthopedic could on 9/16/10. Found small fracture.

EE was helping co-worker tighten up a check curtain in the intake on #2 section. While reaching up to put a nail in the curtain, a small rock hit him on the left side of his face & forced his safety glasses into his face below the eye. A small cut on his cheek was cleaned & a butterfly bandage applied. He later went to see a dermatologist & received sutures for cosmetic reasons.

EE took a lunch break and retrieved his bucket from the first aid car. A rock 3' x 2' x 4" fell onto his right knee. X-rays were negative but made folow-up with specialist on June 3rd, when he received orthoscopic surgery for a torn meniscus. (two staples)

Crew was spot bolting outby face in #2 entry. EE was operating the helper side of a Fletcher RRII. He swung the drill out to spot a bolt between the outside bolt and the rib, when loose rock fell onto his right foot. X-rays revealed fractures to ankle. Surgery on 3/28/10. A plate and screws were inserted. Rock measrued 5"X12"WX30"L. Wearing tiger-tip metatarsal boots.

Member was working in the #4 entry in the intake near the elevator when he tripped and grabbed top brow as he fell, a rock measuring 32" X 21" X 8" thick fell and struck him in the lower back / pelvic area.

Member bent over helping electrician work on miner when a piece of drawrock struck him in the back shoving him into the frame of the machine. The rock measured appx. 3' x 3' and less than 2" thick. Member did not cease work. On May 29, 2009 it was learned that he had seen a doctor and diagnosed with a hairline fractures to four ribs. He said he felt fine and refused to be off work.

Employee just finished bolting 2 right. He removed his gloves, placed his hand on top of the canopy and pulled himself upright to check bolt pattern. A rock, measuring 26" x 10" x 12 - 2 1/2" fell a distance of 18 inches, striking member's right hand. (12 stitches)

EE was bolting top in 3-R crosscut he had lowered his drill steel from the hole and was reaching to get it when a piece of slate measuring 12"Wx31"Lx3" thick fell from the roof stricking him in the left forearm, causing a laceration that required 10 stiches.

Member was in the process of using a slate bar to scale the roof, when as he was pulling a piece of loose roof beside the roof bolter, a piece of top fell and struck his right forearm causing a laceration which needed 12 stitches to close.

Employee was located on the offside of the Fletcher Roof Ranger II when a piece of rock fell from between the last row of bolts & the ATRS. The rock slid down the canopy & struck the EE on the right leg. Rock measured 18"X 14"X 4".

Member was started to drill on roof bolter when a piece of rock striking him on right legs. rock measure 31" long 7" wide 10" thick.

MEMBER WAS LOADING ROCK OUT WHEN A PIECE OF ROCK FELL 12 FT HITTING HIM ON RIGHT HAND CAUSING LACERATION TO HIS SMALL FINGER THAT TOOK 9 STITCHES TO CLOSE

EE WAS CLEANING UP A PIECE OF ROCK BESIDE BELT LINE WHEN A ROCK FELL (MEASURING 2' LONG X 3" WIDE X 0-0.5" THICK) LANDING ON EE'S LEFT HAND, RESULTING IN A LACERATION. 3 SUTURES REQUIRED.

Member was holding a metal wash-down hose. A rock fell striking the hose causing it to flip violently out of his hand, resulting in a broken bone in his right thumb.

Member was standing talking to the boss when a rock measuring 64x38x04" thick fell stirking him on the shoulder.

MEMBER WAS ENTERING SHUTTLE CAR WHEN A ROCK FELL FROM TOP, STRUCK HUNG CABLE, FLIPPED OVER AND STRUCK MEMBER IN THE MOUTH. ROCK WAS 12" LONG, 8" WIDE, 1/2" THICK.

EE WAS IN THE PROCESS OF INSTALLING A ROOF BOLT WHEN A PIECE OF DRAW ROCK WEIGHING APPROX. 3 TO 4 LBS. FELL FROM THE MINE ROOF STRIKING HIM ON THE NOSE.

EE WAS IN THE PROCESS OF POSITIONING BOLTER TO INSTALL FIRST ROW WHEN A ROCK FELL FROM THE TOP OF THE RIB STRIKING EE IN THE CHEST AREA. X RAY STATED A HAIRLINE FACTURE TO HIS RIB.

EE WAS WALKING TOWARDS THE SHUTTLE CAR WHEN A PIECE OF ROCK FELL FROM THE TOP CUTTING HIS RIGHT CHEEK. THE ROCK MEASURED 4' LONG X 7" WIDE AND 1.5" THICK.

EE WAS OPERATING A JOY 14-15 CM IN THE #1 LEFT ROOM WHEN A PIECE OF DRAW ROCK MEASURING 38"L X 28"W X 10" THICK FELL FROM BETWEEN BOLTS STRIKING EE ONT HE HEAD AND RIGHT SHOULDER, KNOCKING HIM TO THE MINE FLOOR. DEGREE 3 CLASS 7.

EE & COWORKER WERE PERFORMING WORK NEAR #5 BELT LINE. AT #42 BREAK WHEN HE NOTICED A 24" LONG X18" WIDE X3" THICK ROCK HANGING DOWN NEAR THE CONTROL LINE. AWAY FROM THE ROCK. THE ROCK FELL PINCHING HIS HAND BETWEEN THE ROCK & BELT STRUCTURE.DEGREE 3 CLASS O7

EE WAS OPERATING A SHUTTLE CAR. HE TRAMMED HIS SHUTTLE TO THE FEEDER AND BEGAN TO EMPTY HIS LOAD. HE GOT OUT OF THE SHUTTLE CAR TO CHANGE SEATS FOR THE RETURN TRIP TO THE CONTINUOUS MINER WHEN A PIECE OF ROCK FELL FROM THE ROOF, STRIKING EE ON THE LEFT ANKLE.

EMPLOYEE WAS IN THE #1-2 CROSSCUT NEAR SPAD #744 LAYING ON THE MINE FLOOR CHECKING ELECTRICAL READINGS IN A CONT MINER PANEL WHEN A PIECE OF ROCK MEASURING 6" BY 1" BY 10" FELL STRIKING HIM IN THE FACE RESULTING IN A LACERATION TO HIS RIGHTEYE.

EMPLOYEE WAS OPERATING A 3 WHEELER AND WAS TRAVELING. BELT LINES BETWEEN 54-1 BELT DRIVE AND THE 48" BELT LINE WHEN A PIECE OF ROCK FELL FROM THE TOP AT 13 BREAK ON 48" BELT LINE STRIKING HIM ON THE HEAD AND SHOULDERS.

EE WAS TRANSPORTING THE EVENING SHIFT CREW TO THE SURFACE IN A BROOKEVILLE MANTRIP. WHILE TRAVELING ALONG THE #18 BELTLINE A ROCK BROW FELL STRIKING THE MANTRIP. A PIECE OF ROCK STRUCK HIM ON THE LEFT LEG AND ANKLE IN THE OPERATOR'S COMPARTMENT.

EE WAS WALKING DOWN TRAVELWAY WHEN A ROCK FELL FROM THE TOP, STRIKING HIM ON THE HEAD AND SHOULDERS.

OPERATOR WAS TURNING 3-4 CROSSCUT WITH JOY MINERWHEN ROCK ABOUT 7 X 7' FELL FROM UNSUPPORTED TOPSTRIKING SHUTTLE CAR & SLIDING INTO OPERATOR PINNING HIM TO THE MINE FLOOR.

INJURED WAS SHOVELING COAL FROM UNDER LEFT CAT PAD ON CONTINUOUS MINER. WHEN HE STOOD UP A ROCK FELL FROM BETWEEN THE LEFT RIB BOLT AND RIB. THE ROCK STRUCK HIM ON THE LEFT SHOULDER KNOCKING H IM TO THE MINE FLOOR. THIS BECAME A LOST TIME ACCIDENT ON NOV 9, 2000. WHEN EE UNDERWENT SURGERY TO REPAIR THE ROTATOR CUFF TO HIS LEFT SHOULDER.

INJURED WAS REMOVING A METAL PANEL FROM A METAL STOPPING WHEN PIECE OF DRAW ROCK BECAME DISLODGED AND STRUCK HIM IN THE HEAD RESULTING IN A LACERATION REQUIRING 9 SUTURES.
