Employee was operating Roof Bolter, spot bolting on the oo1 MMU and a piece of rock fell from the mine roof approx. 2ft. x 1ft. x 2 inches and struck employee on the left side of body on the Ribs and lower back.
Employee was operating roof bolter and as he started drill steel into the roof a piece of draw rock fell striking his left hand index finger requiring 9 stitches in finger.

The employee was taking structure out of the belt when a rock fell out from between the bolts and allegdly struck him on his left hand.
Injured stated he was sitting on the mine floor along the side of the deck of the mobile bridge conveyor he operates when a slab of drawrock fell from between the roof bolts striking him on the head. He was taken to the doctor where he received stitches to his ear and was released.
The EE was bolting, when a piece of rock fell from the top and struck EE in the lower right arm. The rock itself was about 12"wide and 12"long, resulting in nearly a 2" long gash in the EE's arm. EE was taken to the Dr. to have stitches.

INJURED WAS TRAVELING UNDERGROUND ON THE RAIL RUNNER MANTRIP WHEN A PIECE OF DRAWROCK FELL AND STRUCK INJURED ON THE RIGHT ANKLE. INJURED WAS EXAMINED AT THE HOSPITAL AND RELEASED WITH NO BROK EN BONES OR LACERATIONS, OR ANY OTHER SERIOUS DAMAGE TO ANKLE.
THE INJURED WAS OPERATING A CONT. MINER WHEN A SMALL PIECE OF DRAW ROCK FELL AND STRUCK HIM ON THE LT FOOT CAUSING A BRUISE.
