EE stated that he was roof bolting when a piece of draw rock fell striking his right forearm. EE was treated at the ER at Middlesboro ARH where xrays showed no fractures. EE went to the Dr. on 11-3-10 and was admitted to the hospital with cellulitis in the area of the injury.

Showing all 10EE stated that he was standing at the back of the bolt machine as the foreman was checking a test hole in the roof, when a piece of draw rock fell and struck him on the right index finger. EE was taken to the hospital and recieved 13 stitches to the tip of the finger. X-rays showed no fractures. EE returned to work on the next scheduled shift.
EE was installing cable roof bolts, when a small piece of rock appx 6" in dia. X 1/8 " thick fell and struck him on the nose, resulting in 12 stitches to the 1" long laceration.
