Employee lost suction on the drill pot, stepped to the back of tray to check suction, then went back to the canopy when a piece of rock fell, striking them on the right side of ear, neck, shoulder area. They went to ER and needed 3 or 4 sutures on their ear and was released back to work. Accident location #2 entry 001 section 144 feet inby spad 12328.

Employee was replacing a damaged bolt in the #9 entry before bolting the 20' cut, when a piece of rock hit EE in the back. As EE tried to get away another piece of the same rock fell , hit EE in the lower right leg causing a compound fracture. Total size of rock when measured was 4' W X 94" long X 7" thick. Only a portion of the rock struck the employee.

Employee was bolting in the #1 entry on last row of bolts as EE placed EE's starter steel in the pot and pushed it up a piece of bone 15x10x1 inch fell striking EE's right hand causing a laceration requiring 3 stitches

EE was walking down #4 entry to get a roll of fly pad. EE bent over to pick up the fly pad, draw rock fell hitting EE in head and back & knocking EE into the muddy floor. Rock measured 12" wide x 24" long X 11/2" thick, EE was taken to hospital and found to have no broken bones but was taken off work until seen by specialist

Employee was roof bolting the mine roof in # 7 entry when a piece of rock measuring 8 inches long x 5.5 inches wide struck EE on the forearm causing a laceration that required stitches to close back up

employee was taking coal height measurements in #8 heading when a piece of drawrock fell striking employee in the lower back, employee was down on knees getting the measurements when the rock hit them, height in the area was 77". Employee was treated and released to return back to work and worked until 7/13/20 when employee was taking off work

Employee was setting a timber when a small thin piece of rock measuring 4" x 3" fell hitting EE on the right cheek causing a cut resulting in 5 stitches.

EE squatted down to look for an oil leak on the shuttle car, when a piece of draw rock fell striking EE on the hard hat and shoulder. This caused EE to cut EE's forehead. Required seven sutures.

While cleaning on a CM conveyor motor, a piece of draw rock approximately 3" in size fell hitting employee on the wrist resulting in a laceration requiring 6 sutures.

Employee was bolting in the #6 entry when a piece of soapstone or middleman came off striking his left leg, causing a fracture to his knee joint. Rock measured 7 1/2'L X 2 1/2'W x 2"to 6" inches thick.

Employee was B bonding the back side of a stopping at Brk. #33 of #1 entry crosscut of 6 belt when a piece of draw rock measuring 46' x 46' x 2" thick fell out between bolts striking him on the back side of both legs.

Employee was standing beside the roof bolter when a piece of loose draw rock fell hitting on top of the canopy, sliding off the canopy hitting him on the side of the side of his head above his left ear resulting in 14 staples.

Employee was bolting the #9 entry when a piece of draw rock fell from between the inside and outside bolt striking employee on the left foot.

Employee was bolting top when a piece of draw rock fell from the roof when he was pushing his bolt up. The piece of rock glanced off the ATRS boom and then striking employee in the mouth.

Employee was changing hoses on the right roof bolter, offside boom hinge, had hand up on roof bolter when a piece of rock fell from top striking his right ring finger. Roof showed no signs or warnings of draw rock. Rock was approx 2.5ft X 1.5ft X 2-3" thick.

Member had left the section walking to the supply car to get some dust hose for the roof bolter, when a rock fell and hit him on the back. Member worked every shift until 2-3-2011 when he was taken off work due to increased pain in his back and shoulder.

MEMBER WAS CUTTING A HOLE FOR A MOTHER DRIVE WHEN A PIECE OF ROCK FELL & HIT EMPLOYEE ON HIS HEAD, ON TOP OF HIS HARD HAT.

Member was bolting top when a piece of draw rock fell striking him on right forearm.

EE WAS OPERATING MINER WHEN A PIECE OF SLATE FELL STRIKING EE ON LEFT HAND

EMPLOYEE WAS TRAMMING HIS MINER OUT OF THE #2 HEADING WHEN A PIECE OF ROCK 3' X 3' X 4" THICK FELL OUT BETWEEN BOLTS STRIKING HIM IN HIS BACK.

EE WAS PLACING A WRECKED CAR ON TRACK. WHEN A PIECE OF DRAW ROCK 7" LONG 5" WIDE 2" THICK HIT HIM IN THE BACK OF THE HEAD. THIS CUT REQUIRED STITCHES. (NO LOST TIME.)

EE WAS PULLING DRAW ROCK IN LAST OPEN X-CUT OF LONGWALL FACE W/ 5' SLATE BAR, WHEN A PIECE OF DRAW ROCK APPROX. 2' X 1' X 1"-2" STRUCK HIM ON SIDE OF THE HEAD. EE CONTINUED TO WORK UNTIL TUESD AY, OCTOBER 28, 2003 WHEN HE WENT TO THOMAS MEMORIAL EMERGENCY ROOM & WAS DIAGNOSED WITH A CERVICAL SPRAIN. EE STARTED MISSING WORK ON 10-28-03, THAT IS WHY REPORT IS LATE.

EE HAD JUST ARRIVED ON THE #1 SECTION & WAS STANDING IN THE SPAD #2656 MIDDLE OF THE #4 RT. X-CUT ONE BREAK INBY THE FEEDER. WHILE TALKING TO THE SCOOP OPERATOR A PIECE OF DRAW ROCK APPROX. 40 "X 33"X 1-1 1/2" FELL FROM BETWEEN THE MIDDLE BOLTS DRIVING HIM TO THE MINE FLOOR. HE WAS TRANSPORTED TO HOSP. WHERE X-RAYS WERE NEGATIVE. SCHEDULED FOR PHYSICAL THERAPY FOR SORE BACK.

EE & COWORKERS WERE IN THE PROCES OF RECOVERY SHIELDS FROM THE LONGWALL FACE WHEN THE ACCIDENT OCCURRED. AFTER RECOVRING SHIELD 359M THEY WERE IN THE PROCESS OF BUILDING THE SECOND "LINK-N-LOC K" CRIB WHEN A PIECE OF ROCK MEASURING 36" X 36"X 17" FELL. THE ROCK STRUCK THE CRIB THEN HIT EE ON THE BACK & DRIVING HIM TO THE GROUND WHERE HIS RIGHT ELBOW STRUCK THE MINE FLOOR. XRAYS REV

INJURED HAD COMPLETED BOLTING FIRST ROW OF BOLTS IN THE #3 HEADING. HE NOTICED A LOOSE PIECE OF DRAW ROCK APPROX. 33"X23"X3" FELL BEHIND HIM STRIKING HIS LEFT SHOULDER. HE WAS TAKEN TO LOGAN G ENERAL WHERE X RAYS REVEALED A HAIRLINE FRACTURETO THE LEFT SHOULDER.

EE WAS UNLOADING SUPPLY'S FROM A FLAT CAR WHEN A ROCK 3 FT X 4 FT BY 10 IN. FELL FROM BETWEEN THE BOLTS STRIKING HIM ON THE FOOT CAUSING THE INJRY.
