Employee was operating a roof bolter when a piece of draw slate fell striking his right hand resulting in a laceration that required stitches to close.
Employee was operating a roof bolter when he was struck on the shoulder and arm by a piece of roof rock. The piece of rock measured 18 inches by 12 inches by 1-4 inches. The result was a laceration that required 8 stitches.
Employee was operating a continuous miner when a piece of roof rock measuring 4'x 2' 4" fell striking him on the back. He returned to work on the following shift but has continued to have problems with his back. He had a follow up Dr visit on 2/23/09 at which time he was taken away from work. He started losing time on 2/24/09.
ROOF CONDITIONS WERE CHANGING & STRAPS WERE USED. AS BOLTS WERE BEING INSTALLED A PIECE OF ROCK FELL & STRUCK THE EMPLOYEE ON THE LOWER PART OF HIS RIGHT LEG. X-RAYS INDICATED A FRACTURE OF THE RT. FIBULA.
ROOF CONDITIONS WERE CHANGING AND STRAPS WERE STARTING TO BE INSTALLED. AS THE SECOND BOLT WAS BEING INSTALLED IN THE STRAP, A PIECE OF ROCK FELL AND STRUCK THE EMPLOYEE ON THE FOOT. X-RAY DID NOT SHOW A FRACTURE; HOWEVER, AN MRI GIVEN ON 5/27/05 SHOWED A HAIRLINE FRACTURE.
EE WAS HANGING A VENTILATION CURTAIN IN AN AREA OF 8 FOOT HEIGHT. A ROCK APPROX. 18"X12"X2" 20 LBS FELL STRIKING THE EE ON TOP OF HIS HARD HAT. HE SUFFERED A CUT ON THE SIDE OF HIS HEAD APPROX . 2" LONG. THREE STITCHES WERE REQUIRED.

EE WAS STRUCK BY A FALL OF ROCK ON THE ACTIVE 001 RETREAT MAINS SECTION. EE WAS TAKEN TO HOSPITAL AND RECEIVED THREE (3) STITCHES UNDER HIS CHIN. Y
