On 6/24/2008, a piece of draw rock struck the employee's head, neck & back. Immediately following the accident, the employee was treated & released from a local hospital, but has since sought medical treatment. Then, he began taking off work on 5/18/2009 as a result of this accident/injury.

The injured employee was drilling a hole when a piece of rock broke loose from the top, striking the victim's right ear & side of his face. The incident resulted in a laceration to his right cheek that required stitches. The EE's right ear was also lacerated & required stitches.

The employee was starting to drill when a piece of rock fell from the roof, striking the small finger of his left hand, and cutting the tip of it off completely. Per phone call, the original injury did not cut the bone, but the Dr had to amputate just below the fingernail to graft skin around severed part of finger.
