I.E. was having a conversation with another employee at the roofbolter. While standing the roof began to make noise. While getting away from the area, the mine roof fell out and struck the I.E. causing multiple injuries.

I.E. was roof bolting installing their outside bolt. A piece of black shale fell out between the rib and the canopy measuring 1'x2'x2"Th. The rock struck them on the right shoulder. I.E. has continued to work until doctors restrictions took them off on 8-19-2024.

EE was taking down a kennedy stopping. A piece of rock fell from the roof and struck the EE on the right hand. Their hand was was between the rock and kennedy panel. This caused a fracture to their right middle finger. EE continued to work until 2-16-2024.

I.E. was loading bolts on bolter tray when a piece of shale (18"x18"x2"thick) fell from the roof and struck the I.E. in the right forearm. This caused a laceration that required stitches. I.E. has not missed any work.

I.E. was bolting a barrel notch. As they were drilling a hole, a piece of rock fell from the top and struck them on the left forearm. Rock measured 6"x6"x1/4"th. Employee received a laceration that required 3 staples. Employee finished their shift and has not missed work.

I.E. was cutting entry with continuous miner, when a rock measuring (3'x8"x3"thick) fell out from the edge of the screen wire and struck them in the head and shoulder. I.E. has worked full duty until surgery was needed on 2-9-2024.

I.E. just stood up from sitting on the back of the roof bolter to move into a cut to bolt. A rock fell from the roof measuring 5'Lx3.5'Wx3/4-1"th and struck the I.E. above their right eye. Caused a laceration requiring stitches. I.E. has not missed any work.

I.E. was drilling a hole to install a roof bolt. As they were drilling a small piece of rock broke loose and slid down the drill steel. The rock piece was slung from the steel and made contact with their chin. This caused a small laceration that required stitches. Employee has not missed work.

I.E. was driving a coal hauler from the face to the feeder. A rock measuring 3.5' x 1.5' x 1-2"Th fell out from the roof. The rock hit the canopy and a piece of the rock came into the cab and hit the I.E.'s right arm. I.E. has a tendon in their bicep pushed back and a specialist took them off work on 10-18-2021 for treatment.

I.E. was cutting a xcut right with the continuous miner. After setting over for their next lift a rock measuring (1'Wx2"Lx2"Th) fell from the top and struck the top of the I.E.'s right hand. The rock caused a laceration to the top of the hand that required stitches. The I.E. has not missed any work.

I.E. was bolting a crosscut right. While bolting the inside hole on the third row a rock fell out from in front of the ATRS pads. When the rock hit the ground it came back towards the I.E. and struck their left foot. Rock measured 5'x6'x Up to 1' thick. Rock caused fractures in the foot that required surgery. I.E. is a contractor under the mines supervision.

I.E. was repairing the continuous miner. While positioned at the main controller panel a rock measuring 7'Lx3'Wx up to 5 inches thick fell from in between the bolts. The rock landed on top of the CM and then slid down the machine striking the I.E. I.E. received a fracture in one of their vertebrae and a laceration on their head.

I.E. was cutting a crosscut with the continuous miner. A piece of rock measuring 2'x1'x4in thick fell from in between the bolts. I.E. deflected the rock with their arm and it landed on their right foot behind the steel toe causing a fracture in the foot.

I.E. had just advanced the roof bolter to begin the next bolting cycle. As the I.E. started drilling the hole a rock 4'x2'x6" fell out and struck the I.E. in the head. The I.E. was complaining of neck pain. I.E. did not miss any work, reporting due to restricted duty.

I.E. was cutting #2-#3 crosscut with the CM. I.E. heard the top above I.E. making some noise. While attempting to get away from the area a rock measuring 11'L x 4'W x 6"Th came from the roof and struck the employee in the lower part of their right leg causing a tib fib break in the leg.

While roofbolting, a piece of rock fell out on the outside of the screen wire and struck I.E. on the left hand, pinching it between the rock and the bolt tray. Caused a laceration to left ring finger that required 6 stitches. Employee did not miss any work.

Employee was walking to the roof bolter at the face, when a rock fell out between the bolts, striking the employee. Rock measured 5.5'L x 3' W x 1" to 7" thick.

A rock measuring 9'L x 7'W x 3'thick fell out on the left side of a cut at and inby the last row of bolts. The rock struck the I.E. on the left side of the body driving EE to the ground.

Mechanic was on knees working in an electrical panel when a rock fell out between the bolts striking the mechanic on the back of the neck and both shoulders. Size of rock was not approximately determined due to breaking up. Reporting due to multiple chiropractor visits. Employee has continued to work until surgery on 2/5/2020.

I.E. was working on moving up sights for the next cut. As EE moved the line curtain away for EE's laser to advance up toward the face, a rock fell out between the bolts and drove EE to the ground. Rock measured 85"Lx53"wx12-14"th. Evident injuries to right arm and right leg.

While installing additional roof bolts into a sulfur ball, the sulfur ball fell out of the roof, striking the employee's left arm and elbow. Employee received sutures to the left elbow.

Employee was changing a hose and a rock fell out between the bolts and struck left hand, fracturing index finger.

While drilling a roof bolt hole, a piece of rock fell from the roof and struck the employee on the chin. The laceration required 5 sutures.

While cutting coal, a rock fell between the rib bolt and the rib, striking employee on the hard hat and forearm. They received a broken nose and lacerations on the arm and face.

Employee was filling out pre-op/dust parameter sheet when a rock fell out between the bolts and struck ee on the head, shoulders, and/or back. The miner was not running at the time of the accident. EE received a broken nose, and 3 fractured vertebrae. The Accident Hotline was called on 7-27-17, to report on this accident to abate citation # 9104928, Issued on 7-27-17.

The employee's right hand was resting on the canopy of the section service ride when a rock fell striking the employee breaking the trapezoid bone of the right hand between the first and second digit. The employee returned to work the next scheduled shift on restricted duty.

While installing a roof bolt, a piece of rock from the core breaker ridge dislodged striking employee on the upper lip. The employee received 3 stitches and returned to work the same day.

Rock fell and struck employee on the left leg. Fracture to lower left leg.

The contractor was rolling up a line curtain when a rock fell from the roof, striking him in the lower left leg.

Employee was drilling a hole to install a roof bolt, when he saw a rock starting to fall. He turned around hitting his arm on the boom of the roof bolter, causing a 3 in laceration on his right forearm. The cut required stitches.

Employee was behind the line curtain in the number4 room, when a rock fell from between the bolts and rib striking him across his shoulders and back.

Employee was removing Kennedy panels, when a rock fell from the roof, Smashing his left index finger between the support rail and the rock.

A rock measuring 18" x 36" x 1" fell from approx. 7' striking the employee on the head and left shoulder. Employee became a lost time injury on 08/30/2011.

While unloading bolting material out of scoop bucket onto the bolter a rock measuring 30 in x 48 in x 4 in thick fell out between bolt and hit the worker on the left shoulder and on the left hand resulting in a fracture of the left hand.

A rock fell from the roof between the roof bolts in the #4 entry of the 1st E panel striking the employee in the back. This caused him to fall and twist his left ankle. He went to the Dr. during our Christmas vacation and is unable to return to work as scheduled on 1/4/10. Due to the ankle injury.

Was extending the survey sights in #1 room. A piece of rock fell out from in between the bolts hitting him in the head and back. Started missing work on 9/3/08.
