Employee was down on 1 knee cleaning EE's dust box on the roof bolter. A piece of rock fell from between the rib and outside bolt. The rock landed on the back of the foot at the heal. This caused a bruise to the foot.
Employee was swinging in after installing the outside bolt. A piece of rock fell from the top hitting the employee in the right arm causing a laceration that required stitches.
Employee was drilling a test hole when employee put the steel in the chuck, a piece of rock fell and hit employee's left hand between the thumb and index finger.
Employee was operating the boom on the maintenance scoop. A piece of rock fell from the bolt to the rib and struck employee in the back. This caused a bruised muscle.
Employee was bolting in #7 heading, EE swung the pot in to put up the inside bolt. A rock slid off the top of the canopy and hit EE's foot. EE did not miss any work until 6/10/2024. Because of the swelling, EE's boots caused a blister that became infected.
Employee had swung the pot out to put up a corner bolt in the first cut out of a break. As EE was getting on knees, a piece of rock from the first row of bolts outby the cut from the outside bolt to the rib striking the side of EE's leg and ankle causing a fracture.
Employee was drilling a hole to install a bolt in the last row in a cut. A piece of rock fell from the outside bolt to the rib striking employee on the back. This caused a fracture to the employee's spine.
Did not look at roof or pull loose roof
Contract employee had installed first bolt. EE was swinging in to put the inside bolt up. As EE was swinging the boom, a piece of rock hit the canopy, slid off and struck EE's foot. This resulted in a fracture to the foot.
Employee was cleaning off the garbage off of the rear of the roof bolter. A piece of rock fell from the mine roof and contacted their middle finger on the left hand. This resulted in a fracture to the end of the finger.
Employee was walking out by the section and a piece of draw rock fell hitting employee in the head which required staples.
Employee was putting up EE's first bolt in a place. A piece of rock fell between ATRS and the rib hitting EE in the head causing a laceration. this required 5 staples.
Employee was on knees cleaning out the dust box. A piece of rock fell and struck EE on both shoulders and back.
Employee was turning 4R. EE had loaded approx. 5' out of the cut. EE walked up and placed their hand on the miner to look at the cut. A piece of rock fell from between the bolts and smashed EE's left hand causing a hair line fracture to the hand.
Employee had turned out of belt entry into 5 left. Employee said a piece of rock hit canopy, fell off and hit shoulder and neck while employee was tramming the shuttle car.
EE is in the process of training on the roof bolter. EE was trying to get a hole started to install a bolt. EE dislodged a piece of rock and it mashed the tip of EE's pinky finger causing a laceration and fracture.
Employee was beginning to start hole to put the outside bolt up. EE was trying to get the steel started where the top was a little slanted. The starter slipped sideways on the top causing a piece of rock to be dislodged. The rock fell causing a laceration to EE's pinky finger.
Employee was on knees operating a continuous miner. A piece of draw rock measuring 48"x36"x3 1/2" fell from the outside bolt to the rib landing on the bottom of employee's feet, resulting in fractures to both feet.
EE had put outside bolt up, EE leaned over bolter pot to move bolt out of way to swing in to put inside bolt up. Rock fell from top and cut tri-cep of left arm which required 4 stitches
Employee was struck by a rock on hand causing injury
Injured was cutting entry when a piece of draw rock (2" to paper thin) fell from in between rib and bolt striking them on lower back area. The X-Ray and CT Scan taken at ER were negative and employee was suppose to return to work. Another doctor took them off pending MRI.
Employee was putting up curtain board when drill steel got stuck. EE was pulling on steel and board slapped top causing a piece of rock to strike ee on finger pushing it against pot of roof bolter resulting in a laceration.
Employee was using starter steel to start inside bolt. When drill steel contacted the roof a small piece of cap coal fell making contact with EE's left thumb.
Employee was getting off scoop when a small piece of draw rock fell striking EE's finger.
Was at 5 section supply hole getting out of scoop, and a piece of draw rock hit EE in hard hat. Head ache was only complaint. Doctor took EE off for 3 days, and EE has came back full duty.
Employee was walking to roof bolter to move it up when a thin piece of fallen material struck employee on left arm causing a laceration. Employee returned to work on next scheduled work day which was 09/05/2017. Mine was idle from 09/02 thru 09/04/2017 due to Labor Day Holiday.
Employee was setting up roof bolter to bolt last row of bolts, jerked down a small coal brow with T-Bar. It landed on top of the canopy then slid off striking employee's right thumb.
Employee was loading supplies into scoop bucket when a piece of rock fell out from between bolts striking ee on right forearm. This resulted in a laceration which required stitches.
Employee was picking up trash when a small piece of draw rock fell striking employee's right forearm resulting in a small laceration.

He was running a roof bolter. As he put his steel to top. A small piece of draw rock from the steel hit him on forehead causing a laceration that required 7 stitches.
Employee was running a continuous miner on 5 section. He was turning 8 right when a piece of falling material hit him on right hand causing a laceration. He had sutures to close the wound.
EE was waiting on miner when a piece of falling material fell striking him on his middle and ring fingers. 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. Did not know until this time that the ee had a fracture to the tip of his finger, due to the fact that he returned to work the next day to his regular job.
While working on airlock doors EE moved to let mantrip open doors. A piece of falling material fell striking him on back of neck causing him to fall to ground. This is being filed to terminate citation written during a Part 50 Audit. We do not agree this was reportable. We do not accommodate "restricted work activity". Time sheets show EE was back at regular job next day.
Riding up track entry, small piece of falling material fell hitting him on right arm causing a small laceration that required 5 sutures to close.
Injured employee was in the process of installing permanent roof support. He had completed the drilling process and was reaching for the bolt that was laying on the mine floor next to drill pot when a piece of material fell striking his left index finger resulting in a laceration and fracture.
Employee was in the process of installing permanent roof support. He was position next to the tram controls when a piece of falling material struck his right foot resulting in a hairline fracture to his little toe.
Employee had finished installing a roll of roof bolts and had walked back to the dust box, as he was bending and reaching for the straps to open dust box a piece of falling material struck his right forearm causing a laceration. Physician removed him from work due to risk of infection.
While in the process of drilling test hole employee placed his left hand on glue tray, a piece of falling material struck the tip of his left ring finger resulting in a avulsion to the finger tip that required the finger nail to be removed. A physician placed him off work due to risk of infection.
EMPLOYEE WAS RUNNING A CONTINUOUS MINER IN THE #5 HEADING ON 7 SECTION. HE WAS HANGING BACK A LINE CURTAIN WHEN A PIECE OF DRAW ROCK MEASURING 3FT BY 4 FT BY 1 INCH TO PAPER THIN FELL HITTING HIM ON RIGHT KNEE AREA.
Employee was in the process of installing a 10 foot roof bolt when a piece of falling material struck employee's right ring finger resulting in a fracture and laceration.
Employee was riding on a rubber tire 4 wheeler. While traveling outby along 2D travelway, when a piece of falling material fell from the mine roof, striking employee on both legs, resulting in a fracture of the right femur and left knee cap.
