Employee was reclaiming curtains and picking up trash in the left side Entry 2 when a loose rock fell from the top striking the employee's arm. Injuries resulted as an abrasion and strain/sprain to his left arm. Employee will remain off work until released by the doctor on June 06, 2011.
Employee was making up roof bolts at the Main West 1, 64 crosscut, between # 5 and 6 entries (outby), when draw rock fell from between pattern bolts striking employee. The rock measured approximately 7' long, 2.5' wide and 6" to 8" thick. Employee sustained a fractured left forearm, and a torn right patella tendon.
EE was roof bolting on #3 unit,#2 rt. face. EE pryed draw rock with channel locks and rock fell mashing his left thumb on top of canopy. The EE sustained a simple fracture of the left thumb.
EE was operating the continuous miner under supported roof on #4 unit, #4 entry crosscut 58 to 59 when a piece of roof rock dislodged from between the row of roof bolts and the right coal rib. The ee sustained simple non displaced fracture of the right transverse process.
Employee was cutting #7 right at 36 crosscut when rock fell in unsupported roof area and cantalevered into supported roof area striking right ankle. Employee sustained a simple fracture of the right ankle.
Employee was roof bolting when he looked up at hole and piece of rock fell striking lower lip requiring sutures.
The crosscut between #1 & #2 entries had just been cut through. The EE walked up #1 entry to check the bottoms while under supported roof. A piece of roof fell and cantalevered into the support roof, striking the EE, knocking him backwards. EE sustained contusion to left ankle and a hairline partial fracture of T5 vertibre, requiring nonsurgical time off work.
Employee was standing on #1 unit, #3 entry at 16 crosscut when roof rock fell striking ee on left shoulder causing simple fracture of left scapula.
Employee drilled hole in roof on #1 unit in 7-Right when roof rock fell striking him on right leg. He sustained a simple non-displaced fracture in the right knee area.

EE was roof bolting on #4 unit, #8 entry, crosscut 18 when a piece of rock fell, striking him on the neck and shoulders. EE did not sustain any fractures, but missed work on 11/7/06.
EE was standing beside roof bolter when rock fell from between pattern bolts striking him on left shoulder. He sustained a simple fracture of the left clavicle.
EE was standing beside shuttle car when roof rock fell from between bolt and rib, striking his right shoulder. EE missed work on 9/30/06.
Employee was roof bolting when a piece of roof rock fell striking his left foot. Employee sustained a simple fracture to a bone in the left foot.
EE was operating a diesel tractor dragging roadways when a piece of rock fell from between pattern bolts striking him in the left knee. He continued to work until a medical evaluation on 4/27/06 revealed the need for orthascopic knee surgery. This accident became reportable on 4/27/06.
EMPLOYEE WAS WALKING FROM THE FACE TO THE DINNER HOLE WHEN A PIECE OF ROCK FELL FROM BETWEEN PATTERN BOLTS. THE ROCK STRUCK EE ON THE RIGHT LEG CAUSING A LACERATION THAT REQUIRED SUTURES TO CLOSE.
AS EE WAS STANDING IN THE ENTRY A PIECE OF ROOF ROCK FELL FROM BETWEEN PATTERN ROOF BOLTS STRIKING EE IN THE HEAD/NECK AREA. EE SUSTAINED CONTUSIONS TO THE NECK.
AS EE WAS PREPARING TO EXIT A RETURN AIRCOURSE THROUGH A PERSONNEL DOOR, A PIECE OF MINE ROOF MEASURING 33" LONG BY 30" WIDE BY 3" THICK, FELL FROM BETWEEN THE BOLTS AND STRUCK EE IN NECK & HE AD. EE SUSTAINED JAMMED NECK.
