WHILE DRILLING HOLE FOR OUTSIDE BOLT ON SECOND ROW IN 5R COSSCUT A PIECE OF THE ROOF BROKE LOOSEFROM HIS DRILL STEEL TO RIB STRIKING HIM IN THE BACK AND KNOCKING HIM TO MINE FLOOR. THE PIECE STRIKING HIM MEASURED 31" BY 19" APPROX 2 1/2" THICK.
