While re-bolting a steep area where the screen was bagged, the employee grabbed the pan rack with right hand to help climb onto bolting platform. At that moment, a rock fell through screen striking the right middle finger about half way up the fingernail, resulting in the fingertip being amputated, and a tuft fracture of the distal phalanx.

Employee was installing a spot roof bolt. While drilling the hole, a piece of rock fell around the drill steel, striking the employee's forehead and causing a skull fracture and laceration

EE was operating shearer when a piece of coal fell into the bretby trough. EE stopped the shearer from tramming & removed the piece of coal. When returning the shearer, another piece of coal fell from longwall face, striking EE, pushing EE into the shield base lift cylinder.
