Employee climbed into the locked-out longwall pan line, under the protection of a shield, to relieve the ranging arm hydraulic pressure buildup so a broken cutter shaft could be safely removed. A rock fell from the top in front of the shield tip, deflected off the shearer drum, and struck employee in the shin.

Employee was struck on the top of the foot by a stone that fell from the roof. Initial medical imaging was clear and EE was issued a work release for 12/30, EE's next scheduled shift. On 12/26 EE followed up with a specialist who suspected a fracture and placed EE off work pending additional imaging.

The injured employee, an experienced continuous miner operator, was on the off-side of the miner at the bolter station to help troubleshoot why the ATRS jack would not lower. As the IE bent over to look at the hosing a stone fell from the corner and knocked IE against the framework of the miner, causing a laceration to IE's right eye and left shoulder discomfort.

Employee was bolting off the continuous mining machine. Just after EE placed a roof strap in the plank jack, a rock measuring approx. 65" L x 30" W x 12" thick fell from the corner and struck EE in the shoulder area, knocking EE against the dust box of the miner.

Employee was at the control box of a longwall shield and a rock kicked out from the running face and struck EE in the left side near the hip, and EE's left elbow.

Employee was installing roof bolts with another contract employee. They were using a Cobra drill. While drilling, a large rock fell from the top. The rock glanced off the employee's chest then struck the employee's hand, pinching ring and pinky fingers between the rock and bolter pot.

A rock approximately 2' long x 2' wide x 8" thick fell our of the corner in #4 entry at 5 wall (inby left corner) and hit employee in the back, knocking EE to the ground face first.

Employee was walking toward the rear of the miner after finishing putting up a roof strap. Employee had right hand on top of the dust box when a rock fell from between the straps and struck EE's right pinky finger causing a fracture and laceration to the tip of the right pinky finger.

While installing roof screen off of the miner, the top fell in on the roof screen. Employee raised the timber jack to help get the rock off the screen. As the employee and miner operator were pulling the screen back onto the strap, another rock fell, hitting the roof strap. This caused the roof strap to strike the employee in the right side of hard hat.

Employee was helping mechanic service the miner head. As EE was handing the mechanic a ratchet wrench, a piece of rock fell out of the corner of the roof and struck EE in the left lower back.

Employee had EE's arm resting on the bolter tray of the miner. A rock fell out of the top, striking EE's right forearm, causing injury.

Employee was installing cable bolts. Employee drilled up, leaving the steel in the top while holding it up with left hand. Employee was moving the next steel to put it in the drill pot. A rock fell from the top, striking employee's left thumb, causing injury.

Employee was adding a second steel when a rock fell from the binder and caught the left index finger between the rock and the bolter pod.

Rock fell from the top, bounced off the ventilation tubing, and employee was struck by the rock in the nose.

Employee was hanging a ventilation tube & a couple small stones fell from the mine roof. As employee stepped back to look at the roof a larger stone fell & struck employee on the top of the left foot.

Employee was helping install 8 ft. bolts in a outby area and a piece of stone fell & struck EE in the head & neck area.

Employee was standing in an area that had been mined approx. 20 ft. high so an overcast could be built. A rock fell from where roof and rib line meet and struck him on the right side injuring his neck and shoulder.

EMPLOYEE WAS STRUCK ON THE LEFT THIGH BY A 40 TO 50 LB. ROCK THAT FELL FROM ROOF. HE DEVELOPED A BRUISE ON HIS KNEE THAT WOULD NOT HEAL BUT CONTINUED WORKING. ON OR AROUND 06/24/2013 HE HAD TO HAVE MINOR SURGERY ON HIS THIGH TO DRAIN A HEMATOMA. HE WAS PUT OFF WORK TO RECOVER, THUS BECOMING A REPORTABLE INCIDENT.

Employee was installing 2nd bolt in row. Pushing the bolt into the hole through screen, screen was loose and a piece of rock fell (6"x6"x1/4"thick). He turned his head away from it but was struck in the left side of his head and neck.

Employee bent over forward to pick up a slate bar and a piece of loose roof fell hitting him in the back of the head approx. size 24"x10"x1"

Employee states he was fire bossing the 1 Left belt, walking the offside of the belt and was struck by a piece of rock that fell from the top at 14 break on the 1 left belt off side.

Employees were setting timbers when a piece of loose rock fell hitting the employee on the head and back

Employee bent over to remove suction hose from Fletcher single boom bolter. He put his right hand on bolter head to gain leverage to remove hose. A rock measuring 4" X 6" fell from roof and struck employee on top of his right hand middle finger smashing the tip of his finger and causing a laceration requiring sutures.

EMPLOYEE WAS PREPARING TO INSTALL BOLT IN MINE ROOF. AS HE LOOKED OUT FROM UNDER CANOPY TO INSERT RESIN AND BOLT INTO BOLT HOLE, A PIECE OF ROCK FELL FROM MINE ROOF AND STRUCK HIM IN THE FACE LACERATING HIS UPPER LIP, BREAKING OFF ONE TOOTH AND DAMAGING ANOTHER.

Bolter was releasing steel guide and a rock fell and struck his finger.

Employee was standing in a crosscut waiting to bolt when a piece of roof material fell from between straps and struck employee on the right side arm and side.
