Showing all 8Employee was struck by a mill ball that became dislodged on his upper middle back. The employee was given a full duty release from NNRH ER Dr. the day of the injury. At a follow up visit employee was prescribed physical therapy deeming this injury to become reportable as of 06/05/2013.
The employee was cleaning the release handles on a CAT Uni-loader (Skidsteer) with the bucket hanging down and elevated approximately 1 foot off the ground. When the handles released, the bucket rotated forward and dropped off of the attachment plate bouncing off of his right foot and breaking several bones in the top of his right foot.
Employee was drilling out bolt holes in the frame of a dozer using a mag drill. Approx 20 seconds after positioning the drill magnet the drill demagnetized for no apparent reason and fell 12" landing on the employee's left middle finger.
Two employees (welders) were dismantling a large tool rack. The welders were cutting 8'X4'X1/4" plate steel from the frame. The rack became unstable and tipped over on top of the employees. One employee sustained fatal injuries and the other sustained injuries to his back and legs.
While turning on a valve on the water line in the face he was hit on the hardhat with an object that was held by the pipeline. The hardhat came down and struck his nose resulting in sutures.
IW was scraping bed of truck when a chunk of Drycon fell from top of the bed striking her in the elbow.
Employee was struck on the shoulder by a 4" metal ball that fell from a conveyor belt magnet discharge chute.
