Showing all 6On 2-17-21 EE walked up to C3-DC2 to check on dust collector work progress. EE walked near a platform where bags were being lowered and a bag hit their hard hat. On 2-22-21 EE requested to be evaluated due to their neck being sore.
Employee had irritation on back when cement dust fell down the back of EE's shirt on Feb 18. EE decided to get it looked at on Feb 22 and was treated with first aid and released to full duty. On Mar 5 EE went to an Urgent Care clinic and was treated for an infection
On 11-4-15 EE was hit on right shoulder by a wire cutter dropped from above. On 12-7-15 EE stated that the shoulder was sore and was transported to the clinic. EE was diagnosed with impingement syndrome and placed on modified duty and therapy.
While EE was assisting work of another contractor, a 20-lb. cylinder fell from approx. 18" striking EE's right hand and caused a laceration to middle finger.
SUBCONTRACTOR DID NOT ROPE OFF AREA UNDER SCAFFOLD AND DID NOT HAVE ENOUGH PLANKS TO PREVENT A PIECE OF PIPE FROM FALLING APPROX 15 FT. EE WAS WORKING ON INSTALLING GRATES IN THE INCLINE. THE PIPE HIT HIS SHOULDER CAUSING A CONTUSION.
